- The Government intends to award multiple Blanket Purchase Agreements (BPA) for Various Surgical Instruments for the following surgical instrumentation and accessories (see attached list) at the ASFI website: https://acquisition.army.mil. Manufacturer: The Anspach Effort, Inc., 4500 Riverside Drive, Palm Beach Gardens, FL 33410, Phone: 800-327-6887. Calls will be placed against the BPAs as supplies are required. Contractor shall provide all supplies and transportation necessary to Tripler Army Medical Center, Hawaii. Period of Performance is Date of Award through 29 November 2015. NAICS is 339999, All Other Miscellaneous Manufacturing. TELEPHONE REQUESTS WILL NOT BE HONORED.
