Description
This is a combined synopsis/solicitation for commercial items prepared and processed in accordance with the Federal Acquisition Regulations (FAR) Part 12.6 as supplemented with additional information included in this announcement. This is a solicitation from the United States Department of Commerce (DOC), National Oceanic and Atmospheric Administration (NOAA), Eastern Acquisition Division, Kansas City. The requirements are to provide professional medical oversight for 400 Automated External Defibrillator (AED's), for Department of Commerce, NOAA. This notice constitutes the only Request for Quote (RFQ). Written quotes are being requested. A hardcopy written RFQ will not be issued. DOC, NOAA requires that all contractors doing business with this office be registered with the Central Contractor Registry (CCR). No award can be made to a company not registered in CCR. For additional information and to register in CCR, please access the following web site: http://www.ccr.gov. In order to register, all firms must have a Dun & Bradstreet Number. A Dun & Bradstreet number may be acquired free of charge by contacting Dun & Bradstreet on-line at https://www.dnb.com/product/eupdate/requestOptions.html or by phone at (800) 333-0505. Contractors should submit their Certification and Representations through the on-line Business Partner Network (BPN). The BPN is the single source for vendor data for the Federal Government. Contractors are required to go on-line at www.bpn.gov to complete its representations and certifications. This notice is hereby issued as NAAJA000-11-00195. This RFQ and the following incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 05-46. The line items will be quoted and completed in accordance with the terms and conditions and specifications specified herein. Prices quoted for contract line items shall be firm-fixed-price inclusive of all costs for completing the entire job as outlined in the statement of work (SOW): LINE ITEM 0001: Provide the work and services necessary for the medical oversight of 400 AED's located at various NOAA Offices in the United States, Territories and on NOAA ships and aircraft for a one year period. 400 Ea Unit Price $_________ Total Price $_________ LINE ITEM 0002: Option Period One will be for a one year period, beginning 365 days after award of the purchase order. Provide the work and services necessary for the medical oversight of 400 AEDs located at various NOAA Offices in the United States, Territories and on NOAA ships and aircraft for a one year period. 400 Ea Unit Price $_________ Total Price $_________ LINE ITEM 0003: Option Period Two will be for a one year period, 365 days after award of Option Year One. Provide the work and services necessary for the medical oversight of 400 AED's located at various NOAA Offices in the United States, Territories and on NOAA ships and aircraft for a one year period. 400 Ea Unit Price $_________ Total Price $_________ LINE ITEM 0004: Option Period Three will be for a one year period, beginning 365 days after award of Option Period Two. Provide the work and services necessary for the medical oversight of 400 AED's located at various NOAA Offices in the United States, Territories and on NOAA ships and aircraft for a one year period. 400 Ea Unit Price $_________ Total Price $_________ LINE ITEM 0005: Option Period Four will be for a one year period, beginning 365 days after award of Option Period Three. Provide the work and services necessary for the medical oversight of 400 AED's located at various NOAA Offices in the United States, Territories and on NOAA ships and aircraft for a one year period. 400 Ea Unit Price $_________ Total Price $_________ Total Price for all line items: $________ Statement of Work Every Line/Corporate Office that has an AED program is required to meet federal, state, and local guidelines for medical oversight of their program, throughout the United States, and its Territories. NOAA will provide many of the services for its public access defibrillation program, but needs a vendor to complete the AED program. The vendor will provide the following services: 1. Physician's Prescription: AED's are regulated by the Federal Drug Administration (FDA) and require a physician's prescription to purchase. The vendor will provide unlimited prescriptions as needed for the purchase of AEDs and must have physicians licensed in all fifty states and all U.S. Territories. 2. Complete Medical Oversight: The vendor will provide complete compliance under federal, state, and local government regulations regarding the NOAA AED program. Complete Medical Oversight will allow NOAA's AED Program to be turn-key. The services included in the package will be: Initial Physician Prescription Ongoing Medical Direction Under a Licensed Physician Review of NOAA's AED Program Policies & Procedures Manual Send event information to the local Emergency Medical System (EMS) office Notification & Communication. Provide quality assurance and quality control of site placement of AED's by telephone. Monthly Maintenance Reminder (by Email) Use NOAA's AED Tracking System Program to Monitor Expiration Dates of Electrode Pads & Batteries In the Event of AED Usage, vendor will provide: a. Post-Event Review and Data Management under the supervision of a licensed physician b. A loaner AED will be shipped free with next day delivery while the data on the event from the AED is being downloaded and interpreted. c. Data is sent to requesting parties per FDA requirements d. NOAA National AED Coordinator receives copies of all reports. e. FREE Shipping of our AED & loaner AED both ways In sum, the vendor will provide the above services to ensure a complete joint Medical Oversight package, regardless of AED location which will include Aircraft, Ships, Small Boats, and Facilities throughout the United States, and its Territories. Also, the vendor will provide NOAA with all new protocols as specified by the American Heart Association (public access defibrillation program). 3. AED WEBSITE CREATED BY VENDOR - The vendor will create an easy to use website that tracks NOAA AED's. The website will allow NOAA to add, edit, and review program data and alerts. The vendor will also notify NOAA Site Coordinators, when batteries and/or electrode pads are nearing expiration. In addition, the website will allow NOAA's National Coordinator to monitor NOAA's employees recertification due dates and monitor all AED usages. In addition the website will provide the national coordinator whatever reports are needed. Another required feature will be the ability to add AED program notes and reminders. This service must include a 24-hour toll-free customer service and support line. NOAA personnel must be able to test the Web site AED within 10 days after award of the contract. NOAA will provide the following : NOAA's National Coordinator Responsibilities: • Coordinate Medical Emergency Response Team (MERT) members, and distribution of MERT voluntary member lists as required. • Coordination of CPR/AED/First Aid training, for MERT voluntary members, by local EMS providers as applicable. • Coordination of equipment, accessory, and maintenance of AED equipment with local manager. • Review of this policy and procedures on an annual basis. • Communication with medical director, VENDOR, physicians on issues related to medical emergency response program including post event reviews. Applicable Documents: • AHA, and CASA AED guidelines • NOAA AED Procedure • NOAA Bloodborne Pathogen Exposure Control Plan • State AED legislation NOAA On-Site Manager: The on-site AED Coordinator is will be provided upon award. • Responsibilities of the on-site coordinator include assuring that the AED is maintained in a state of readiness. • Complete the monthly inspection of all assigned AED equipment, and document. • Assure continued competency of the authorized individuals trained to use the AED. • Ensure that local EMS Agency is notified of any change in on-site manager. Maintaining Readiness: The AED will be checked by NOAA for readiness after each use and at least once every 30 days if On Site Coordinator and/or designee have not used it in the preceding 30 days. Authorized Users: The following individuals have successfully completed training: Volunteer Responders: All other office personnel will receive in-service on the AED. A refresher in-service will be provided on an annual basis by NOAA. Quick Reference Cards: The Quick Reference cards will be kept posted next to the AED cabinet and will include information on the following: • Procedure for use of the AED • Steps to follow after using the AED • AED monthly checks Acceptance will be at destination. FOB point is destination. The selection resulting from this request for quotation will be made on the basis of the lowest priced technically acceptable quote from responsible quoter who meets the minimum contractor qualifications shown below. Once the Contracting Officer has decided to whom an order will be issued, a unilateral order will be issued using an Optional Form 347. A purchase order will be in existence once the Contracting Officer has signed the order. Wherever the words offer, proposal, offerors, or similar terms are used in this solicitation or in any applicable provision, they shall be read to mean quote, quotation, quoter, vendor or similar corresponding term to reflect that this solicitation is a Request for Quotation not a Request for Proposal or an Invitation for bid. Provision 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. The Contracting Officer will evaluate quotes on the basis of information furnished by the quoter that demonstrates that they exhibit an understanding of the requirements as identified in the quote and reasonably available to the Contracting Officer. The vendor is required to submit information that describes the services they will be performing for the medical oversight, and two past performance references that will demonstrate the vendor has successfully provided the required services. The Contracting Officer is not responsible for locating or obtaining any information not identified in the quote. Telephonic inquiries will not be accepted.
