- Service Contract Act Wage Determination Class evaluation form Forest Service Class Roster BLM Class Roster Safety class ordering form Instructions for Submission of Offers Solicitation This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in the Federal Acquisition Regulation (FAR), Subpart 12.6 as supplemented with additional information included in this notice. Solicitation number AG-04N0-S-11-0001 is issued as a request for quotation (RFQ). This action is set-aside 100% for small business. The North American Industry Classification System code for this procurement is 611699. The purpose of the contract is to secure a performance-based, fixed price, single award contract for the period of January 1, 2011 through September 30, 2013. This contract will be ordered as a base year with 2 options years. The services being procured are for Health and Safety Training Classes. All classes will be held in the Central Oregon Area including Crescent OR to Paulina OR and points-in-between. The Contractor shall provide all sufficiently skilled labor that meets the requirements listed in the course descriptions, training materials, mileage/transportation, lodging, and operating supplies to perform the work in compliance with this contract. The Contractor shall furnish laptop computers, workbooks, projectors, manikins, face shields and any other equipment that is needed to present these trainings in a favorable learning environment. The Contractor shall be required to submit a course syllabus for each class that is being offered. The Contractor shall submit resume's for every instructor that they are proposing to offer for fulfillment of this contract. The Contractor shall provide a contingency plan that will describe the method planned by the Contractor to replace an instructor in the event of an emergency, illness or death.
