Description
CMS determined that a cost-reimbursement type contract is most appropriate for this procurement because in addition to conducting general requirements in accordance with the MIDS contract the Contractor shall conduct an ad hoc analysis for which the actual measures are not yet known. The Contractor must develop these measures using clinical data collected by registries and anticipate the use of data elements derived from the Electronic Health Records (EHRs) systems that will be employed by hospitals in the future. The use of existing clinical databases from registries presents a risk to the Contractor because which databases will be used is not known and the databases may not allow tracking using a common identifier. This may require the Contractor to match the patients in a registry with those in the Medicare database and change CMS' desired outcome of developing measures that can be applied to all-payor populations, not just the Medicare population. The contractor must also develop the measures as consistent as applicable with those already developed by CMS. This will require an unknown amount of coordination with CMS contactors responsible for other readmission measures. The Contractor must also submit the measures to the National Quality Forum (NQF) for endorsement and provide technical support during the process. Additional analyses to revise the measure methodology as requested by the NQF may be required. The fact that the clinical data and data elements are not well defined and that it is not possible for the Contractor to accurately define the amount of time it will spend during development, coordinating effort and participating in the endorsement process with NQF increases the Contractor's risk substantially making it unreasonable to expect the Contractor to provide a fair and reasonable firm-fixed price. Thus, it is in the best interest of the Government to award this Task Order as cost plus fixed fee (CPFF).
