- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation W91YU0-11-T-0005, (WC1JUG02794446 and WC1JUG02544500) is being issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-46 The associated North American Industrial Classification System (NAICS) code for this procurement is 339112 (Surgical and Medical Instrument Manufacturing ), Size standard is 500 employees. This RFQ is being issued as Firm Fixed Price, 100% SMALL BUSINESS SET ASIDE All responsible sources may submit a quotation which shall be considered by the agency. These items/supplies are for use at Bassett Army Community Hospital, Fort Wainwright, AK 99703 All responsible Contractors shall provide an offer for the following. Brand Name or Equal. Contractor will need to provide a complete breakdown on the contents in the seven pouches. CLIN 0001. Pediatric Resuscitation System-Fully stocked, Part Number: AE-4700, 14 each qty; Unit issue: EA CLIN 0002 Freight Charges qty: 1 Unit issue: LS Specification "Includes twenty-eight size-specific procedural modules in seven pouches, flying carpet, Broselow" Pediatric Emergency Tape, and all other equipment shown in photo. "All contained must be in a heavy-duty ALS equipment organizer. "The Broselow Pediatric Emergency System is approved for the PALS education program. For children 3 to 36 kg. "Must have the capability to fill multiple vials from a single prescription (when the quantity of meds is larger than the one vial size can hold.
