- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. This solicitation is issued as a Request for Quote (RFQ). ProVideo Color Cone Vision Testing System is a computer software-generated clinical color vision test that indicates type (red, green or blue) and severity (mild to severe) of color vision deficiency and quantifies normal color performance. The color vision testing system was developed under a Cooperative Research and Development Agreement between the United States Air Force School of Aerospace Medicine and Innova Systems, Inc. The Government requires 266 testers to be shipped to 266 medical facilities and clinics throughout the world. The solicitation document and incorporate provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-46 (effective 21 September 2010). FAR 52.212-4 and 52.212-5 applies to this acquisition. WAWF procedures also apply. FAR 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. FAR 52.212-2, Evaluation - Commercial Items, applies. It is the offeror's responsibility to be familiar with applicable clauses and provisions. The delivery date of these items is 60 days after receipt of Order (ARO), FOB Destination, 266 locations to be identified. The NAICS Code is 339115 the size standard is 500 employees. The 711th Human Performance Wing/PK, Brooks City-Base TX intends to award a firm-fixed price purchase order by other than full and open competition in accordance with Subpart 6.302-1 (c). Only one responsible source and no other supplies or services will satisfy agency requirements. The supply classification code is 65.
