- SYNOPSIS DESCRIPTION: The Department of Veterans Affairs, Veterans Integrated Service Network (VISN) 19 Rocky Mountain Network Contracting Center 4100 East Mississippi Avenue, Suite 610, Glendale, CO 80246 intends to solicit sources to enter into a single or multiple firm-fixed price Basic Ordering Agreement(s) (BOA) to provide Adult Day Care Service for the following regional VA Medical Centers: Denver, CO, Grand Junction, CO, Fort Harrison, MT, Cheyenne, WY, and Sheridan, WY. Services shall be for a one year base period with four one-year pre-priced option periods for renewal. The solicitation package should be available for download on or about October 09, 2010. Proposals submitted shall be due no later than 4:30 p.m. mountain time on November 01, 2010. This solicitation is unrestricted and is subject to full and open competition. The applicable NAICS code is 624120 with a small business standard of $7.0 million. The contractor shall provide comprehensive adult day care to include the following: medical and nursing services, social and rehabilitation services, nutrition, recreation, transportation (to and from client's residence) and case management services. The contractor shall be able to demonstrate technical expertise and previous performance history in the area of adult day care as part of their proposal. The contractor(s) shall be responsible for all applicable professional, state and facility licensure and accreditation. The contractor shall have established adult day care facilities in the catchment area serviced by the above listed VA Medical Centers. The catchment area for the FORT HARRISON VAMC includes the following counties: MONTANA COUNTIES - Beaverhead, Big Horn, Blaine, Broadwater, Carbon, Carter, Cascade, Chouteau, Custer, Daniels, Dawson, Deer Lodge, Fallon, Fergus, Flathead, Gallatin, Garfield, Glacier, Golden Valley, Granite, Hill, Jefferson, Judith Basin, Lake, Lewis and Clark, Liberty, Lincoln, McCone, Madison, Meagher, Mineral, Missoula, Musselshell, Park, Petroleum, Phillips, Pondera, Powder River, Powell, Prairie, Ravalli, Richland, Roosevelt, Rosebud, Sanders, Sheridan, Silver Bow, Stillwater, Sweet Grass, Teton, Toole, Treasure, Valley, Wheatland, Wibaux, Yellowstone; NORTH DAKOTA COUNTIES - Billings, Golden Valley, Slope. The catchment area for the EASTERN COLORADO HEALTH CARE SYSTEM (DENVER) VAMC includes the following counties: COLORADO COUNTIES - Grand, Summit, Lake, Chaffee, Park,,Clear Creek, Gilpin, Boulder, Jefferson, Douglas, Denver, Broomfield, Adams, Arapahoe, Elbert, Morgan, Washington, Yuma, Kit Carson, Mineral, Saguache, Rio Grande, Conejos, Costilla, Alamosa, Custer, Fremont, Teller, El Paso, Pueblo, Otero, Bent, Kiowa, Cheyenne, Prowers and Baca; KANSAS COUNTIES - Cheyenne, Sherman, Wallace, Logan, Greely, Hamilton, Stanton. The catchment area for the CHEYENNE VAMC includes the following counties: WYOMING COUNTIES - Albany, Carbon, Converse, Goshen, Laramie, Niobrara, and Platte; NEBRASKA COUNTIES - Banner, Cheyenne, Kimball, and Scottsbluff; COLORADO COUNITES - Larimer, Logan, Morgan, and Weld. The catchment area for the SHERIDAN VAMC includes the following counties: WYOMING COUNTIES - Albany, Big Horn, Campbell, Carbon, Converse, Crook, Fremont, Goshen, Hot Springs, Johnson, Laramie, Lincoln, Natrona, Niobrara, Park, Platte, Sheridan, Sublette, Sweetwater, Teton, Uinta, Washakie, Weston; MONTANA COUNTIES - Big Horn, Yellowstone. The solicitation reference is VA-259-10-RP-0107 - Adult Day Care and should be referenced in all correspondence/inquiries.
