This price is determined fair and reasonable based on historic pricing and average cost of contracts currently in place at the VA Medical Center. 8.Market Survey (FAR 10): In accordance with FAR Part 10, a source sought was conducted, only one VOSB responded. 9.Other Considerations: N/A 10.Listing of Sources that expressed, in writing, an interest in the acquisition: Samaritan, Veolia, Alternate Transportation Systems, Alert Ambulance Service, New England Ambulance Services, and Coastline Ambulance Services. 11.Statement of Actions to Overcome Barriers to Competition: This is an interim contract to allow services to continue while competing a long term contract. 12.Requirements Certification: I certify that the requirement outlined in this justification is a bonafide need of the Department of Veterans Affairs and that the supporting data under my cognizance, which are included in the justification, are accurate and complete to the best of my knowledge and belief. ____//es//__________________________9/27/2010__________________ Name: Kimberly ChoplinDate Title: Contracting Officer Facility: Providence 13.Approvals in accordance with FAR 6.304 ____//es//_________________________9/27/2010_____________________ Name: Kimberly M.
