- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The USPFO for Kentucky has a requirement for Physical Health Assessments (PHA) to be performed at various locations. Locations and Specifications can be found in the attached Statement of Work (SOW). This solicitation will be awarded to the Service Disabled Veteran Owned Small Business (SDVOSB) with the lowest price technically acceptable. Evaluation criteria will include relevant past performance, technical, and Price with all being equal. Contract Line Item Numbers (CLINs): 0001, PHA/Dental Evaluation event Greenville KY, for up to 575 Soldiers. 0002, PHA/Dental Evaluation event Morehead KY for up to 400 Soldiers. 0003, PHA/Dental Evaluation event Louisville KY for up to 67 Soldiers. Contractor shall provide, all management, labor, materials, equipment, certifications and supplies necessary to perform all required medical and dental evaluation tasks in accordance with Army Regulations and the Statement of Work. Include a separate price for blood type testing per test as required. 0004, Army Contractor Manpower Reporting Requirement (yearly requirement). Include price for this reporting requirement (see Statement of Work) or indicate No Charge. Closing date for this RFP will be 14 October 2010 at 2pm local time.
