A4: 4 AMENDMENT TO COMBINED SYNOPSIS SOLICITATION Solicitation Number:W81XWH-10-T-0632 Customer:Medical Communications for Combat Casualty Care (MC4) Post Date:09/30/10 Response Date:10/7/10 Contracting Office Address: US Army Medical Research Acquisition Activity (USAMRAA) ATTN: MCMR-AAA-E, 820 Chandler Street, Frederick, MD 21702-5014 Solicitation This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The U.S. Army Medical Research Acquisition Activity (USAMRAA) is seeking the following: TRUCK, LIFT 2-3 TON (commonly known as a forklift); Quantity = 1. Specification: See attachment. This requirement also includes maintenance on the leased forklift. Duration = Base year with four one-year options. Partial quotes will not be accepted. Item shall be shipped directly to Medical Communications for Combat Casualty Care (MC4) Warehouse, currently located at Tobyhanna Army Depot, Tobyhanna, PA. This announcement constitutes the solicitation by USAMRAA, Ft Detrick, MD as a Request for Quote (RFQ) and offers must be submitted in writing (oral offers will not be accepted). The solicitation document and incorporated Provisions and Clauses are those in effect through Federal Acquisition Circular (FAC) 2001-22. All firms or individuals responding must be registered with the Central Contractor Registration (CCR). This will be a small business set-aside. The SIC Code is 2320 and the North American Industry Classification System (NAICS) code is 425120.
