Description
This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number VA-262-10-RQ-0594 is issued as a request for quotation. This solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-43. This is a small business set-aside. The associated NAICS code is 423490 and the Small Business size standard is 100 employees. Line Item 1: Research Grade EEG System $_______________________ (1) EACH Offerors must include warranty information and a list of specific items that constitute the system. This system will be used for research in the fields of schizophrenia, traumatic brain injury, post-traumatic stress disorder and depression. The system must use low-impedance electrodes with amplifiers located inside the electrode to provide the highest quality EEG recordings with the least amount of noise. The application of electrodes must be accomplished quickly and comfortably without skin preparation (abrasion of the scalp). The system must have integrated channels to record physiological responses, such as electrophysiological (EEG/ERP) and psychophysiological (heart rate, EMG, skin conductance) functioning, in schizophrenia and schizoaffective patients. Delivery of the equipment shall be FOB Destination to the VA San Diego Healthcare System, Warehouse/Receiving, 3350 La Jolla Village Drive, San Diego, CA 92161, within consignee's premises, 90 days after receipt of order. At the time of delivery, all deliverables shall be new, free from defects and in good working condition. Re-manufactured equipment shall not be accepted. Offerors must comply with all instructions contained in provision 52.212-1 Instructions to Offerors - Commercial Items. Pursuant to FAR Clause 52.212-4, para (t), Central Contractor Registration (CCR), after initial registration the contractor is required to update registration data as changes occur and must re-register annually to ensure that all data remains current. Noncompliance with this requirement will preclude the exercising of any option periods that may be included herein and will be cause for termination of the contract at such time noncompliance is discovered. Refer to cited clause for more details. You are required to register with the CCR database at the following web address: CCR - http://www.ccr.gov. It is imperative that you complete this registration in order to be considered for contract award. In accordance with FAR Provision 52.212-1 paragraph (j), please provide the Dun and Bradstreet Number assigned to your firm in the space provided: DUNS# __ __ - __ __ __ - __ __ __ __. In accordance with FAR provision 52.212-2 Evaluation-Commercial Items, the Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: 1. Describe how your system reduces noise to produce high-quality recordings. 2. Describe the process used to prepare electrode recording locations and attach electrodes. 3. Describe your system's capability to record physiological responses. All evaluation factors, other than cost or price, when combined are significantly more important than cost or price. 52.212-3 Offerors Representations and Certifications - Commercial Items - The contractor must return a completed copy of provision 52.212-3 with the offer. A copy of the provision may be obtained from https://www.acquisition.gov/Far/. 52.212-4 Contract Terms and Conditions - Commercial Items (10/03) applies to this acquisition. 52.212-5 Contracts Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items.
