- SOLE SOURCE LETTER This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is FM442702651226; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-43 effective 2 Aug 10, Defense DCN 20100927, and AFAC 2010-0903 effective 03 Sep 2010. This acquisition is SOLE SOURCE-MUSCULOSKELETAL TRANSPLANT FOUNDATION; the North American Industry Classification System (NAICS) code is 339112. The business size standard is 500 employees. The Federal Supply Class (FSC) is 6515. The Standard Industrial Classification (SIC) is 3841. The requirements are as follows: CLIN0001- 6CM X 16CM FLEX HD ALLODERM ALLOGRAFT, QTY-4 Award shall be made in the aggregate, all or none. The following provisions and/or clauses apply to this acquisition. Offerors must comply with all instructions contained in FAR 52.212-1 Instructions to Offerors -- Commercial Items.
