Description
Statement of Work (SOW) Early Intervention & Transition Assistance for OEF/OIF Combat Veterans & Their Families Background: Public Law 110-387, section 302 requires VA to conduct a three year pilot program, in coordination with the Vet Center program, to provide readjustment services and transition assistance in helping combat Veterans and families cope with the challenges associated with the transition from military to civilian life. VA plans to implement this program as a natural extension of the Vet Center mission to promote access and early intervention through outreach and preventive education. The program will target rural and minority combat Veterans in the State of Washington; who are at greater risk due to geographical and cultural barriers to access. The primary objective of this program is to assist Navajo Native American veterans who served in a combat zone with the active military service, reserve and/or Navajo Nation National Guard to overcome obstacles to care through provision of Veteran peer services that are culturally competent. Scope of Work: The contractor will provide all plant, labor, materials, and equipment necessary to provide outreach services, referral, and information necessary for the Navajo Native American soldiers, sailors and airmen of the active forces, the National Guard and Reserves to receive the highest level of service and benefits available to the Veteran. The services and deliverables as required in this SOW shall be carried out in close coordination with the Navajo Vet Center. These services are intended to have a significant positive impact on the lives Navajo Native American combat Veterans. The contractor will be required to interact with Military, Reserve, National Guard, Community Agencies, and Service Organizations as well as Vet Center staff. Basic policies and procedures are outlined in M-12, Part I & II and the Vet Center Policy Manual. This provides overall direction at the Vet Center. However, due to the nature of the services and the unique considerations of the veterans of Navajo Native Americans in WA, the contractor may be required to develop additional procedures. Definitions: OEF/OIF - Operation Enduring Freedom/Operation Iraqi Freedom http://www.oefoif.va.gov/ COTR - Contracting Officer's Technical Representative Specific Requirements or Tasks: Task 1: Establish culturally appropriate outreach counseling for benefit services, transitional assistance services, and assisted program briefings. These services shall be specifically tailored toward the Navajo Native American Veterans to include Military, National Guard and Reserve Units providing transitional assistance information and briefings. Task 2: Identify eligible Veteran personnel and provide them with information on Vet Center and VA Services and benefits. Task 3: Engage in a variety of community outreach activities that will promote knowledge and use of Vet Center services for outreach services and preventive education services to OEF/OIF combat Veterans and family. These activities will include (but are not limited to) such items as public speaking, professional presentations, information booths, liaison with community and government agencies, interviews with the media and personal telephonic contact with the target population. Task 4: The contractor shall make referrals to the Vet Center for follow-up readjustment counseling services as indicated during an outreach event. Task 5: The contractor shall, to the maximum extent possible, furnish Veteran staff to fulfill the requirements of this pilot program. Task 6: Contractor shall conduct orientation training for participating staff from non-VA agencies. This orientation training shall be conducted immediately following award of contract and concurrently with the initial phase of pilot implementation. Training will include curriculum topics on combat Veteran readjustment problems, military-related family readjustment problems, military history in the OEF/OIF combat theaters, and service needs of special Veteran populations. Training will also focus on the Vet Center program mission and promote the building of liaisons between participating agencies and area Vet Centers. Deliverables: Assessment reports will be required every two months. These reports will document the outcomes of the pilot program, and will contribute to the overall program assessment report submitted to Congress. The reports shall include: 1: a description of the activities under the pilot program as of the date of each report, including the number of Veterans and families provided assistance under the pilot program and the scope and nature of the assistance so provided; 2: a current assessment of the effectiveness of the pilot program; and 3: any recommendations for the extension or expansion of the VA pilot program. While the Tribe retains the rights to information gathered as a sovereign entity they agree not to use any of the data for any other purpose than what was intended under the contract. ?
