Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation # VA-241-10-RQ-0659 is issued as a request for Quotation. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2001-27. This solicitation is unrestricted. North American Industry Classification Code (NAICS) 711130 applies to this requirement and the size standard is 7 million. The government intents to award a firm fixed price contract under Simplified Acquisition Procedures (SAP). Period of performance, which will occur at TOGUS VA Medical Center Chapel, ME 04330, shall be from 1 October 2010-30 September 2011. CLIN 0001, Base Year, 111.00 EA Organ performances for October 1st, 2010 to September 30th, 2011 CLIN 0002, Option Year 1, 111.00 EA Organ performances for October 1st, 2011 to September 30th, 2012 CLIN 0003, Option Year 2, 111.00 EA Organ performances for October 1st, 2012 to September 30th, 2013 CLIN 0004, Option Year 3, 111.00 EA Organ performances for October 1st, 2013 to September 30th, 2014 CLIN 0005, Option Year 4, 111.00 EA Organ performances for October 1st, 2014 to September 30th, 2015 The contractor will serve as a Catholic and Protestant organist for the TOGUS VA Medical Center. He/she will have full responsibility for providing musical performance and accompaniment in both the Catholic and Protestant services (i.e. 26 per quarter “ 2 each Sunday) as well as not to exceed 7 special performances annually (e.g. special Catholic services and special Protestant and/or Ecumenical services) for a total of 111 performances annually. The 7 special performances are: Holy Thursday, New Years Day, Four Chaplain Remembrance Service, National Day of Prayer, Veterans Day, and 9-11. Key Requirements include U.S. Citizenship, Knowledge of both Catholic and Protestant music and liturgy, one year of professional experience equivalent to the level which has demonstrated both practical musical knowledge and skill in a pluralistic setting, educational degrees in music are appropriate, but not required. In addition, offeror must have ability to read music (i.e. not play only by ear). An authorized representative of the offeror must sign these representations and certifications; (4) The Offeror shall also provide its Commercial and Government Entity (CAGE) code, Contractor Establishment Code (DUNS number) and Tax Identification number.
