Description
STATEMENT OF WORK Community Living Center Contract Proposal - I recommend a Massage Therapy Contract be established as follows: Scope and Purpose of the Contract Description of the work - Massage Therapist will provide therapeutic chair massage to CLC residents, including Hospice residents at end of life. This is specialized service that Type of Services - Skilled professional to provide massage to reduce stress and requires licensing and specialty area certification. agitation via chair massage. Activity increases comfort and relaxation and contributes to overall functional improvements. Proposed Contractor - Jane Owens of Keep in Touch Massage. Ms. Owens has worked in CLC over one year. Contract Structure - Contractor will provide eight (8) hours/week = 416 hours per year. Financials - $60 per hour times 416 hrs = $24,960 Start Date - October 1, 2010 thru September 30, 2011 (Contract is 5 years/1 Base year plus 4 option years. Describe - Contractor will: oProvide ten minute chair massages = 8 hours/week x 52 weeks = 416 hours. oBe responsible for documenting notes regarding resident contacts and forwarding these to Recreation Therapist. oBe responsible for taking attendance each session. oProvide education to staff and veteran regarding massage as needed or requested. oRequest 30 days in advance any scheduled time off. oOther duties related to recreation/leisure programming as requested by Recreation Therapist. VA will provide chair and massage lotion. Recreation Therapist will monitor programs including veteran response and documentation. Lunch, breaks and invoice preparation are not included in contracted hours. General Information FTE:.2 FTEE *NOTE: In the case where hours are not provided as per contract (i.e. contractor illness, needs of residents) unscheduled hours within the allotted contract will be made up on evenings and/or weekends.
