- NOTICE OF INTENT The Department of Veterans Affairs, VISN 7, intends to award a sole-source service contract to LABORIE MEDICAL TECHNOLOGIES CORP, 400 Avenue D, Suite 10, Williston, VT 05495-7828 to provide priority and unlimited phone support ((8) hours a day, five (5) days a week, not including holidays) for technical assistance related to software, hardware and how to issues of the AQUARIUS TT URODYNAMIC SYSTEM located at the Veterans Affairs Medical Center (VAMC) in Augusta, GA 30904-6285. Vendor will also provide for modem support to dial in to the system and make modifications in setup, study software, or just general system operation. Minimum one year service shall include but not limited to the following: Provides unlimited telephone support for technical assistance related to software, hardware, and how to issues of Urodynamics. Provides unlimited modem support. Able to dial-in to the system and make setup modifications, study software, free software upgrades, and general system maintenance. Provides loaner equipment at no charge, if a loaner is available, if a loaner is not available the repair will be placed on a priority repair. Provides all repair parts and labor at no charge. Company s discretion to utilize refurbished or new parts. Provides free software upgrades to equipment which has been deemed eligible by LABORIE MEDICAL TECHNOLOGIES CORP as eligible for upgrade. Provides one (1) visit per contract year, the customer may reserve a) An in-service by a clinical consultant or, b) A preventative maintenance by a field service engineer or c) Used for a repair visit. Interested persons may identify their interest and capability to respond to the requirement or submit proposals. This notice of intent is NOT a request for competitive proposals or a solicitation for offers. All capability statements received by closing of this notification of this synopsis will be considered by the Government. A determination by the Government not to compete based on responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. This acquisition is being conducted under simplified acquisition procedures. At this time, there are no set-aside restrictions for this requirement. The intended procurement will be classified under North American Industry Classification System (NAICS) 423450 with a Small Business Size Standard of 100 employees. However, parties interested in responding to this notice shall follow the instructions below: POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e., small/large) and DUNS Number. 2) Provide a Statement of Capability that demonstrates the offeror's past performance in providing service to the AQUARIUS TT URODYNAMIC SYSTEM. Please include the following: (a) staff expertise including their availability, experience, and formal and other training; (b) current in-house capability and capacity to perform the work; (c) prior completed projects of similar nature; (d) corporate experience and management capability; and (e) examples of prior completed Government contracts, references, and other related information. 3) Submit Capability Statements by September 15, 2010 to: Department of Veterans Affairs, VISN 7 Network Contracting Activity ATTN: Juan A. Jusino, Augusta, GA 30904-6285. Responses should be received no later than September 15, 2010 at 2:00 PM EST. Submissions shall be made via email to: juan.jusino@va.gov or FAX: (478) 274-5771. NO CAPABILITY STATEMENTS MAY BE SUBMITTED AFTER SEPTEMBER 15, 2010. AT THIS TIME NO SOLICITATION EXISTS. (DO NOT REQUEST A COPY OF THE SOLICITATION). A FUTURE NOTICE WILL BE POSTED WHEN THE SOLICITATION WILL BE RELEASED. 4) Disclaimer and Important Notes: This notice does not obligate the Government to award a contract or otherwise pay for the information provided in response. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate.
