- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a combined synopsis/solicitation for commercial non-personal services prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This action is a 100% small business set-aside. The applicable North American Industry Classification System (NAICS) code is 621610 with a size standard of $13.5 million. Payment will be made by the Defense Financial and Accounting System (DFAS); therefore, you must be registered in the Central Contractor Registration (CCR) (http://www.ccr.gov/), as well as, Wide Area Work Flow (WAWF) (https://wawf.eb.mil/). Award shall be made to the most technically acceptable, best value proposal. You shall submit commercially available literature that demonstrates you are able to meet all of the requirements specified in the attached Performance Work Statement. From the FAR, the following provisions and clauses are applicable to this solicitation: FAR 52.212-1, 52.212-3, 52.212-4, 52.217-8, 52.217-9, 52.204-7, 52.204-10, 52.237-7and 52.212-5 with applicable items 52.219-6, 52.219-28, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-36, 52.232-33 and 52.222-53. From the defense Federal Acquisition Regulation Supplement (DFARS), the following provisions and clauses are applicable to this solicitation: DFARS 252.212-7000, 252.204-7004 Alt A, 252.232-7010, 252.232-7007, 5152.209-4000 and 52.212-7001 with applicable items 252.203-7000 and 252.232-7003. This requirement is a non-personal services contract as defined in FAR 37.101, under which the contractor is an independent contractor as a full-time licensed Nurse at White Sands Missile Range (WSMR), NM. The contract will be for one base year plus three option year for the services specified under the attached Performance Work Statement. The Government will provide office space, computer, and office supplies. Your proposal information and commercial literature shall be submitted electronically to jennifer.brown20@us.army.mil or by facsimile to 575-678-5796 Attn: Jennifer Brown no later than 5:00pm MT 16 September 2010. Please use the attached Proposal Summary as a template for your priced proposal. Proposals shall include your Tax Identification Number (TIN), Dun & Bradstreet D-U-N-S number and pricing for the base and option year. Price proposals should be valid for 30 days from 16 September 2010. Evaluation of proposals will be based on the attached Evaluation Specifications criteria. Proposals will be approved ONLY if the contractor meets ALL of the minimum requirements specified in the Performance Work Statement. Do not submit a proposal unless ALL of the minimum requirements are met at the time the proposal is submitted (16 September 2010). Along with the proposal summary, please provide a resume (companies that submit proposals MUST identify specific personnel currently working within the company and provide their resume and contact information). Technical proposals shall identify exactly how all of the minimum requirements are met preferably at the top of the page in a list as seen in the Performance Work Statement (i.e. requirements a - h listed under III Requirements paragraph 2).
