- Price Schedule Wage Determination Statement of Work Clauses and General Provisions Articles and Special Provisions 1. The 902D Contracting Squadron, Randolph AFB, TX, is seeking person(s) to operate a Massage Therapy Services concession inside the Rambler Fitness Center located on Randolph AFB in accordance with the attached Statement of Work (SOW). This announcement constitutes the only solicitation; quotations are being requested and a separate written solicitation will not be issued. Solicitation number RANDF012300030 is issued as a Request for Quotation (RFQ). This requirement is unrestricted. The associated North American Industry Classification System code is 621399 with a size standard of $7,000,000.00. This combined synopsis/solicitation is NOT to be construed as a commitment by the United States Air Force. The Government is NOT liable for any costs and/or activity associated with providing a response to this announcement. Any cost incurred as a result of a response to this announcement shall be borne by the firm and will not be charged to the Government for reimbursement. 2. The Clauses, General and Special Provisions, Articles (Concessionaire Contract), Wage Determination, Statement of Work, and Price Schedule applicable to this RFQ and the resultant contract are attached. Federal Acquisition Regulation (FAR) clauses can be viewed full text through the Internet FAR site: http://farsite.hill.af.mil. 3. Specific Requirements: Massage Therapy services for a period beginning on or about 1 October 2010 through 30 September 2011 with four (4) possible extensions/option years. Those desiring to submit an offer must possess a valid massage therapist license and submit proof of same with their quotation. A minimum two (2) years experience is required. Additionally, provide not less than one or more than three (3) references to include names and phone numbers, with the quote. 4. The Concessionaire will pay the customer - the Non-Appropriated Fund Instrumentality (NAFI) - a monthly fee (minimum $200.00) to be entered on the attached Price Schedule. The greatest fee amount proposed, in conjunction with an evaluation of qualifications/references will determine the awardee of subject concession. 5. Interested parties capable of meeting this requirement MUST submit a written quote to include the company or individual name, complete address and phone number, tax identification number (TIN), and DUNS and CAGE code, if applicable. Oral quotes will not be accepted. All responses that meet the criteria contained within this announcement will be considered. Written quotes may be e-mailed, faxed, or hand-carried to arrive at the 902D Contracting office not later than 16 September 2010, 4:00PM Central Time. Late quotes/offers will not be considered. 6.
