Description
STATEMENT OF WORK Community Living Center Contract Proposal - I recommend a Recreation Therapy Assistant be established as follows: Scope and Purpose of the Contract Description of the work - Recreation Therapy Assistant will provide support to the Recreation Therapist to include planning and coordination of activities. Taking attendance and input event captures. Clerical support to include typing and advertising activities. Activities will be age- appropriate and diverse in nature to meet the interests and needs of Community Living Center (CLC) residents. Tvpe of Services - Support of recreation/leisure program for CLC residents including hospice. Knowledge of geriatric population needs and those with functional and cognitive impairment. Proposed Contractor - Diane Quig. She has worked in CLC for over one year. Contract Structure - Contractor will provide twenty (20) hours/week = 1,040hours per year. Financials - $22 per hour Start Date - October 1, 2010 thru September 30, 2011 (Contract is 5 years/I Base year plus 4 option years). Describe - Contractor will: oDemonstrate knowledge of therapeutic recreation design, application and theory oComplete attendance sheet and event captures as directed. oAssist in transporting residents and implementing program. oGreet new residents and initiate "Getting to Know You" form. oCoordinates educational programs for veterans including research of project. oDemonstrate ability to adapt activities to residents cognitive and functional level. oDemonstrate knowledge and ability in applying age-specific competencies. oEngage residents in games and activities. oRequest scheduled time off thirty (30) days in advance of leave. oOther duties related to recreation/leisure programming as requested by Recreational Therapist. oComply with Information Security rules and regulations. oComplete annual training requirements. Recreation Therapist will monitor programs including veteran response and documentation. Lunch, breaks and invoice preparation are not included in contracted hours. Materials to be used: Provided by VA. General Information FTE:.5 FTEE *NOTE: In the case where hours are not provided as per contract (i.e. contractor illness, needs of residents) unscheduled hours within the allotted contract will be made up on evenings and/or weekends.
