Section B - Supplies or Services and Prices ITEM NO SUPPLIES/SERVICES QUANTITY UNIT UNIT PRICE AMOUNT 0001 12 Months #70150-001 FFP Lease of ThinPrep T3000 Processor, to include instrument service and maintenance. See Section C for Statement of Work. FOB: Destination MILSTRIP: N0018311RQLA004 PURCHASE REQUEST NUMBER: N0018311RQLA004 NET AMT Section C - Descriptions and Specifications CLAUSES INCORPORATED BY FULL TEXT THE MINIMUM REQUIREMENTS OF THIS ORDER ARE AS FOLLOWS: • Vendor to provide lease of Thin-Prep T3000 Processor. (see listing below for minimum item / quantity requirements) • Items Required: Part # Item Quan 70150-001 Thin-Prep Processor 12 mo. o In the event of technology upgrades vendor may substitute equal or better items / equipment. The Government shall maintain the final determination of deciding equal / better. The decision of equal / better shall be made by the Director of the Laboratory/Cystology or an authorized agent. o Vendors price to be inclusive of all cost including: parts, labor and transportation including Shipping. • Supplied items shall be warranted to be free of manufactures defects. o Faulty items and or items damaged through normal operation, shall be repaired or replaced as soon as possible but later than 5 business days after notification. o Vendor shall not be responsible for damage as a result of abuse from Government personnel. o All equipment shall remain the property of the vendor. o Vendor shall be responsible for total maintenance of supplied equipment. NMCP shall be responsible for routine maintenance such as cleaning. Maintenance records shall be available for inspection by the Government as soon as possible after request but no later than 2 weeks after the request. All equipment to be maintained per OEM guidelines. In addition, NMCP shall be responsible for replacement of damaged and lost items as a result of abuse or misuse. o All items shall be kept / stored on-site at NMCP proper. All items will be made available to the vendor for inventory purposes with a 24-hour advance notice. Upon request (24 hours in advance) the vendor shall be given the right to inspect storage location of items. o Vendor shall not be held responsible for damages or harm caused by the miss-use of any equipment covered by this agreement. o Vendor must supply a written consent prior to any assignment of rights or obligations resulting from this agreement. • Equipment supplied must be certified for use in US Hospitals adhering to UL and or JACHO requirements as required.
