- REQUIREMENTS LIST LIMITED SOURCE JUSTIFICATION This is being issued as a Request for Quote (RFQ), this solicitation document and incorporated provisions and clauses are those in effect in the current Federal Acquisition Regulation (FAR) FAC 2005-43 and Defense Federal Acquisition Regulation Supplement (DFARS) DCN 20100820 that are included in each of the awarded contracts. There are additional DFAR clauses added to this request below. The NAICS code is 334111 and the small business size standard is up to $1,000 Employees. The Business Transformation Agency (BTA) has a Limited Source requirement for One Service Maintenance Agreement Renewal for Polycom Video Teleconferencing Equipment in support of the BTA Information Technology (IT) Department in accordance with attachment one(1). This requirement is being awarded under the CHESS Contract a DoD Indefinite Delivery/Indefinite Quantity contract. Requirement shall be delivered on-site and via internet as required. The delivery date is no later than 10 days after receipt of award. FOB point is Destination. Inspection and acceptance shall be at Destination by the Government. The following additional DFAR clauses apply to this acquisition and shall be incorporated into any task order: The clause at DFARS 252.204-7004, Required Central Contractor Registration; DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Statues or Executive Orders; DFARS 252.225- 7001, Buy American Act and Balance of Payment Program. All firms must be registered in the Central Contractor Registration database @ www.ccr.gov. The above clauses can be found at http://farsite.hill.af.mil. Quotes shall be evaluated on a lowest price technically acceptable basis.
