- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. A Computerized matt Cutter with attachments is being solicited by the Nebraska Army National Guard as a combined synopsis/solicitation for commercial items IAW Subpart 12.6,as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number, W91243-10-T-0025 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 10-1. Notice is given that this is a Total Small Business Set-A-Side; the associated NAICS code is 333293 and the small business size standard is 500 employees. (v) The following list of contract line item number(s) and items, quantities and units of measure are required to be responded to and identified within the quotation submitted. CLIN 0001: 1 EA - Mat Cutter, Z13FCMC25 Mat-ProiX CMC 150 OR EQUAL, 48 inch x32 inch includes Green cutting head; windows PC; 17 inch monitor, Keyboard, 1-L100 Blades, instruction manual, future mat designer software an Horizontal Stand. CLIN 0002: Cutting Head - 1 EA, 'BLUE' 45 degree Bevel 'Cut from Back'; CLIN 0003: 1 EA, Drawing Head, Mat Stylus Deluxe Drawing Head; CLIN 0004: 1 EA, Embossing Head, 'Violet' with 3 tips; Freight charges are required to be submitted as a separate line item if separate for purposes of evaluation and payment. Requirement shall be delivered within 30 days FOB Destination to the Nebraska Army National Guard, 2950 North Park Road, Lincoln, NE 68524. The provision at 52.212-1, Instructions to OfferorsCommercial, applies to this acquisition. No addenda is provided. The quotes shall evaluated based on the product that most accurately meets the requirements of the items stated for the intended purpose. Price shall be equal in importance to the evaluation of the specified product. Offerors are advised to include a completed copy of the provision at 52.212-3, Offeror Representations and CertificationsCommercial Items, with its offer. Clause 52.212-4, Contract Terms and ConditionsCommercial Items, applies to this acquisition. No Addenda applies. Clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive OrdersCommercial Items, applies to this acquisition. The Buy American Act applies to this order as well. The offeror shall provide a three (3) year extended warranty with the product offered. Offeror shall be required to provide initial installation and 1 time training to a minimum of 3 personnel onsite at the location of the installation of the equipment.
