COMMERCIAL COMBINED SYNOPSIS/SOLICITATION This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number 445609 and issued as a Request for Quotation (RFQ). Submit written offers only; oral offers will not be accepted. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-44. This acquisition if unrestricted. The Northern American Industry Classification System (NAICS) code is 423810 and the size standard is 100 employees. DESCRIPTION OF LINE ITEMS: Asphalt Crack Sealer (Melter Applicator) The following are this solicitation's list of contract line item number(s) and items, quantities and units of measure: 1 (one) Asphalt crack sealer 200 gallon material capacity, engine capacity suitable for material capacity heated hose applicator engine cover, engine gauges, spare tire, locking battery box, safety hooks, fire extinguisher, pintel hitch, compressor with minimum 70 cfm, hydraulic surge brakes. DELIVERY: The date for delivery is 45 days after order received. The place of delivery and acceptance is 1301 Fifth Street; Susanville, CA 96130. Shipping terms are FOB Destination. SUBMISSION OF OFFERS: The provision "52.212-1, Instructions to Offerors-Commercial", applies to this acquisition and the following are provided as an addenda to this provision. Paragraph (b), entitled "Submission of Offers", Request for Quotation should be submitted on company letterhead with DUNS number and TIN number. The pricing information shall contain the following information: Solicitation Number; Name; Address; Telephone Number of Offeror, Price; Any Discount Terms, Response to non-price evaluation factors and Acknowledgement of all Solicitation Amendments. Quotes must indicate quantity, unit price and total amount. Offers may be submitted via facsimile or mail. All offerors must provide a completed copy of "52.212-3, Offeror Representations and Certifications - Commercial Items" or must have completed their annual representations and certifications electronically at http://orca.bpn.gov. DUNS Number: TIN Number: ___________________ EVALUATION FACTORS: The Government will award the contract to that responsible vendor whose quotation, conforming to the combined synopsis/solicitation, will be most advantageous to the Government, considering price and the price-related factors described below. Factor One - Price Factor Two - Vendor's availability to perform and deliver tractor by specified date Factor Three - Vendor's applicable past performance and experience Price and non price evaluation criteria are equal. In addition to pricing, the vendor shall submit information on their ability to supply the Asphalt Crack Sealer (Melter Applicator) and a proposed delivery schedule. Offerors should furnish a reasonable number of references for past performance. The Government may contact references to inquire: (1) that the offeror was capable, efficient, and effective; (2) the offeror's performance conformed to the terms and conditions of its contract (specifications); (3) finished within the contract time; (4) the offeror was reasonable and cooperative during performance; and (5) the offeror was committed to customer satisfaction. Past performance information that will be considered is not limited to the references provided by the offeror.
