- Performance Plan Performance Work Statement Past Performance Questionnaire RFP Solicitation Notice Travis AFB intends to procure a full-time medical radiation dosimetrist at David Grant Medical Center, Travis AFB, CA. Period of performance is a base period of 1 Oct 2010 - 30 September 2011 with four option years running concurrently to the fiscal year through 30 September 2015. Complete details of the Dosimetrist services required, evaluation factors, and the Performance Work Statement (PWS) are included. This requisition is solicited as a 100% small business set-aside. The North American Classification System (NAICS) is 621512 and the related size standard is $11.5M. Award shall be made based upon the best value to the government in accordance with Federal Acquisition Regulation (FAR) 52.212-2 Evaluation of Commercial Items. The government reserves the right to award a contract to other than the low offeror. The government will not provide paper copies of the solicitation package. Facsimile, telephone, written and/or email requests for the solicitation package will be honored. Prospective offerors who download the solicitation are requested to submit their company name, address, phone number, fax number, email, points of contact, Cage Code, DUNS number and business classification (i.e. - large, small, disadvantaged, 8(a), HUBZone, women owned, veteran, service disabled, etc) to Cynthia Littlejohn, Contract Specialist, at cynthia.littlejohn@us.af.mil. All responsible sources may submit a proposal which shall be considered by the agency. This notice does not obligate the government to award a contract nor does it obligate the government to pay for any proposal preparation costs. Prospective offerors must have or obtain a DUNS number, be registered with the Central Contractor Registration (CCR) at http://www.ccr.gov, must complete FAR 52.212-3 Offeror Representation and Certification Commercial Items at http://orca.bpn.gov, and Wide Area Work Flow (WAWF) at https://wawf.eb.mil to ensure electronic funds transfer.
