- (i)This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in subpart 12.6, in conjunction with the policies and procedures for solicitation, evaluation, and award as prescribed under FAR 13.1, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will NOT be issued. The proposed contract action is for a Heart Mapping system for the VA Medical Center Minneapolis, MN 55417. This equipment must meet the minimum requirements as described in Section (vi). (ii) The solicitation number is VA263-10-RQ-0537 and is issued as a Request For Quotation (RFQ). (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-43, July 23, 2010. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at http://www.arnet.gov/far (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR). (iv) This solicitation is issued as full and open competition. The North American Industry Classification System (NAICS) code is 339112, and the size standard is 500 employees. (v) This requirement consists of three (3) line items: CLIN (1): CARTO 3 Heart mapping system which includes operator and service manuals. Quantity 1; CLIN (2) CARTO 3 workstation cart Quantity (1); CLIN (3) Arm, patient interface unit, Quantity (1); (vi) The CARTO 3 heart mapping system should include the software license, location pad with foot pedal, a grounding cable, optic cable, a power supply, a patient interface unit, a pin box, system cables, a patch unit, a Windows workstation with two 24 inch high resolution flat screen repeater monitors, users manual, a color inkjet printer and staff training. This solicitation will require the inclusion of security language from VA Handbook 6500.6, located at https://www.voa.va.gov/DocumentView.aspx?DocumentID=228. The awardee will be required to agree with and act upon the following sections of that handbook: 1. General, 2. Access to VA Information and VA information Systems, 3. VA Information Custodial Language, 4. Information system design and development-paragraphs (g,h,I,j,k,l), 5. Information System Hosting, Operation, Maintenance, or Use “ paragraphs (f,g,h), 6. Security Incident Investigation, 7. Liquidated Damages for Data Breach, 8. Security Controls Compliance Testing, 9. Training. The procurement of these items will require the awardee to sign a Business Associate Agreement (BAA). It will also require a low risk (NACI) background investigation for all contracted employees working on site at the MVAHCS. The offer shall specify technical specifications and proposed adaptive equipment and provide estimated number of days to deliver. (vii) Delivery FOB Destination to VA Medical Center, One Veterans Drive, Minneapolis, MN 55417. The Government will award a firm fixed price contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, technical factors combined with past performance is equal to price.
