- SOURCES SOUGHT NOTICE This is NOT a solicitation for proposals or quotes. The purpose of this notice is to obtain information regarding: General Statement of Need: The availability and capability of qualified business sources that can provide a Thermoformer which is a fully automated prosthetic socket forming machine, designed to use industry standard socket preforms for fast, consistent results. Sockets formed from preforms are generally stronger and have less shrinkage than sockets that are formed from flat sheets. The Thermoformer product as listed herein, as requested by authorized VA Staff, in order to support the Jamaica Plain campus of VA Boston Healthcare System (VABHS), 150 South Huntington Ave, Boston, MA 02130. The equipment provided under this contractor shall be F.O.B. Destination; fuel surcharges shall not be allowed nor added to the invoice. The Thermoformer features and specifications are listed below: 1. A one piece spun aluminum oven for efficient heating, built-in ring and table storage; 2. An illuminated forming area; 3. Clamp rings and table for 16, 18, and 25 cm performs; 4. Monitors door position, carriage position, oven temperature, oven over-temperature, drive motor load, drive speed, and vacuum system function; 5. Carriage travel only occurs when the door is closed; 6. Locking casters; Welded stainless steel construction with easily removable front panels for service access; 7. Service manual; 8. 2-year warranty. Specifications are: 1. Capacity: 9.7 diameter x 23.5 (25 cm diameter x 60 cm) 2. Overall size: 26 x 31.5 x 85 (66 x 80 x 216 cm) 3. Weight: 440 lbs (200 kg) 4. Power: 230 VAC 15a, 50/60 Hz 5. Preform Sizes: 16, 18, and 25 cm 6. Oven: 230 degree C max 7. Vacuum: 27 Hg 8. Controls: Perform Selection, Temperature, Time, Start, Up, Down, Stop, Add Time 9. Display: 20 character x 4 line back lit LCD displays set temperature, actual temperature, vacuum, heat or vacuum time remaining.
