Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice for the South Texas Veterans Health Care System. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. This procurement is being issued as a Request for Quotations (RFQ), and the reference number is VA-257-10-RQ-0268. This solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular 2005-43B, 2005-44. The NAICS code is 423450 and the size standard is 100 employees. The government intends to purchase 10 (KT) of the following: Respironics V60 Non-Invasive Ventilator, Mobile O2 Configuration includes (Model #1062628): Universal Roll Stand, V-60 Mounting Plate, V60 Configuration Kit, C-Flex Option, AVAPS Option, Ramp Option, Support Arm, Circuit Support Arm Bracket, Mounting Kit for Humidifier, Oxygen Hose, Oxygen Module, 6 Hr. Li-ion Long Life Battery, Universal Roll Stand Oxygen Cylinder Kit, Oxygen Tank Manifold, Quick Reference Card, and Hospital Grade Power Cord. All equipment will include two (2) days (8 hours per day taking into consideration shift work employees) of on-site installation and training. All travel must be in accordance with the Federal Travel Regulations contained in 41 Code of Federal Regulations (CFR), Chapters 300 through 304. The contractor agrees to furnish two copies of a manual, handbook or brochure containing operating, installation, and maintenance instructions (including pictures or illustrations, schematics, and complete repair/test guides as necessary). The system shall include a warranty for a minimum of one year by manufacturer or in accordance with commercial industry standards. The Government anticipates awarding a single firm-fixed price contract to the contractor providing the best value to the Government. Technical elements are significantly greater than the price element. (1) Technical: Product Features; a copy of specifications for other than brand name product. (2) Warranty: Please include proof of warranty. (3) Price: Quote for each item. (4) Installation and training: FOB destination. All equal products must be accompanied with manufacturer's salient characteristics, brochures, and literature or other information that will enable a clear technical capability analysis. ALL OFFERORS MUST HAVE CURRENT ONLINE REPRESENTATIONS AND CERTIFICATIONS APPLICATION (ORCA) OR ARE TO INCLUDE A COMPLETED COPY OF THE PROVISIONS AT 52.212-3, OFFEROR REPRESENTATIONS AND CERTIFICATIONS COMMERCIAL ITEMS WITH OFFER. The following FAR clauses and provisions are applicable to this solicitation: 52.212-1, Instructions to Offerors-Commercial Items, FAR 52.212-2 Evaluation-Commercial Items, 52.212-4, Contract Terms and Conditions--Commercial Items; 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercials Items; The following FAR clauses identified at paragraph b of 52.212-5 are considered checked and are applicable to this acquisition: section (b) (1),(4), (19-26), (34), (39); 52.211-6, Brand Name or Equal, 52.247-34 F.O.B. - Destination; 52.214-21-Descriptive Literature; 852.211-73 Brand name or equal, 852.211-70 Service Data Manuals, 852.2246-70-Guarantee, 852.273-76-Electonic Invoice Submission, 852.246-71-Inspection, and 852.270-4 Commercial Advertising. Quotes shall be submitted on company letterhead No Later Than (NLT) 3:00 PM September 7, 2010, via the addresses provided above based on delivery method or facsimile.
