WB4AA-0152-1004 A. GENERAL INFORMATION C3-RC, Bldg 60, Rock Island, IL has a requirement for Safety Eyewear TF Bulldog, FOB Blessing, AFGHANISTAN. This is a streamlined combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This notice constitutes the only notice. A written solicitation will not be issued. This requirement is issued as a request for quote and all quotes will be evaluated as ALL or NONE for purpose of award using lowest cost technically acceptable evaluation method. A Firm Fixed Priced award will be made in accordance with the following the Schedule of Supplies: 1.DESCRIPTION/QTY: ITEMDESCRIPTIONQTYUIUNIT PRICE 1 Safety Spectacles/eyeglasses: This is a Brand name only RFQ: OAKLEY SI Ballistic M Frame 2.0 Strike Array; Safety Spectacles/eyeglass Kit; Frames must meet Mil Spec Standard, MIL-PRF-31013 and ear piece must be wrapped in rubber like material (Unobtanium), provide snug fit during WET conditions thus preventing spectacles from slipping off head during environmental high temperatures that cause human body to perspire excessively. Lenses must meet ANSI Z87.1 standards for HIGH/Ballistic Level impact resistance. Spectacles/eyeglasses shall be on the US Army; Authorized Protective Eyewear List (APEL). Kit must contain different lenses that are interchangeable with single black or OD Green frame. Lenses must be; clear, gray and laser protective and VR28 to provide UV protection but allow others to observe wearers eyes and all lenses provide maximum scratch resistance. Kit shall contain prescription lens carrier (PLC) frame attachment that meets APEL. Kit shall have carrying case that prevents sand/dust intrusion. Kit shall have cleaning cloth and may have cleaning solution included. Vendor should be able to provide replacement lenses or frames/parts upon request at an additional cost. Vendor shall provide standard commercial 1 year warranty. All vendor claims shall be independently verified through US Army Medical Command. Due to the high volume of requested item: Volume discount is highly encouraged. QUANTITY REQUIRED: 750UNIT OF ISSUE: KT Cost:$__________________________ SHIPPING Cost:$__________________________ 2.BRAND NAME JUSTIFICATION: The US Army has thousands of soldiers deployed throughout Afghanistan. The average temperature ranges from 100F to 120F degrees. The US Army units in Afghanistan field tested numerous brands of spectacles and found OAKLEY eyewear best supports their needs. Oakley Ballistic M Frames are designed to tightly hug the head, and the earpieces are wrapped in a rubberlike material called Unobtanium. This material withstands the extreme temperatures and the subsequent heavy perspiration, holding the eyewear in place during mission execution. Other brands did not provide that fit or stability. Other brands slipped off soldiers faces, resulting in obscured vision and loss of eye protection during combat actions. The VR28 lens is on the APEL but on the ground US Army Commanders have approved this lens for cultural sensitivity. The VR28 lens provides the necessary eye protection and allows Afghan leaders to observe US soldiers eyes during vital and sensitive meetings. US Army Field Commanders require the maximum eye protection for their soldiers and after field testing all known brands of APEL safety eyewear; Oakley Brand is the only one that provides that requirement. 3. Supplies are for delivery FOB destination. b. Required delivery NLT 30-days after award. c. Award will be made on a lowest priced technically acceptable approach and in the best interest of the Government. d. Provide GSA, Tax ID, Cage Code, & DUNS. e. Contractor is liable for the ACCURACY of submitted quote(s) and for any incurred costs if quote(s) was submitted erroneously. B.
