- Price Exhibit to be submitted together with signed copy of Amendment 0004 Revised Performance Work Statement (PWS) dated 25 August 2010 for Amendment 0004 Amendment 0004 to re-open RFP. PWS has been revised. See page 13 ofattached PWS and Price Exhibit for Amendment 0004. Closing 31 August 2010, 1100 A.M., Japan Standard Time (JST). Amendment 0003 is issued to provide revised Performance Work Statement (PWS) and Government responses to Contractors' questions. FAR 13.5 has also been included. Closing date has been extended to Monday, 16 August 2010, 11:00 A.M., JAPAN STANDARD TIME (JST). Contractors having access to Kadena Air Base are asked to drop in their proposal packages on the said closing date and time duly date stamped in the Bid Box located at Bldg. 99, 18 CONS/LGCA. Interested contractors without access to Kadena Air Base are requested to meet at Gate 2 Pass Office on the closing date 16 August 2010, 10:50 A.M., JST to submit their proposal packages to Rieko Toma. Revised Performance Work Statement (PWS) has been provided. PWS (page 13), 4.11 SPECIAL QUALIFICATIONS has been revised to read as follows: 4.11. SPECIAL QUALIFICATIONS. The contractor shall have Qualified Pool Maintenance Personnel with at least three (3) years of verifiable experience in pool operation and maintenance related work. Awarded contractor shall posses a current pool operation and maintenance management personal License issued by the Japan Pool Amenity Association and complete the swimming pool hygiene manger personal training through Japan Pool Amenity Association PRIOR 01 OCTOBER 2010 - before starting work under this contract. Helpers or apprentices shall be permitted to work under the supervision of a certified personnel. The contractor shall furnish a list of all personnel and their qualifications at the time of proposal submission for review by the CO. Contractors that had submitted proposal packages on 16 August 2010 are requested to re-submit the proposal with the attached price exhibit of Amendment 0004 and signed copy of Amendment 0004 of this RFP. All other interested contractors are to submit the whole proposal package. The required response date/time has changed from 16-Aug-2010 11:00 AM to 31-Aug-2010 11:00 AM.
