- Description
- The Department of Veterans Affairs, North Little Rock, AR (VISN 16), is issuing a solicitation for Fabrication and Repair of Dental Fixed and Removable Prosthesis for the Central Arkansas Veterans Healthcare System. Augmented dental services may also be requested. Service will require that the contractor provide all materials and supplies necessary to complete each item. The contractor shall provide items that meet the quality, esthetis and functional requirements of the patients and all dentures and partials will carry a patient identification. Service will require daily pickup and delivery of impressions, teeth, prescriptions, finished products, etc. Monday through Friday (working days), 9:00 a.m.-3:00 p.m. (working hours), excluding Federal Holidays. Completed work shall meet the schedule of deliverables and be returned to CAVHS Dental Service within the established timelines identified in the statement of work. This solicitation is issued as a commercial item in accordance with FAR Part 12 and shall be evaluated in accordance with FAR part 13.5 Commercial Items - Test Program. The RFP VA-256-10-RQ-0164 will be advertised as 100% Small Business set aside under NAICS Code 339116 with a size standard of 500 employees. Only offerors from those firms regularly established in business of providing this type of service will be considered. This solicitation will be advertised and available for download on the Federal Business Opportunities website in conjunction with this Synopses. Offerors are advised they are responsible for obtaining any and all posted amendments. Any and all questions regarding this solicitation must be in writing on company letterhead to include telephone number, fax number and point of contact to the Central Arkansas Veterans Healthcare System, Contracting Office, Claudette Barbee, Contracting Officer, Bldg 41, Room 202, 2200 Fort Roots Drive, North Little Rock, AR 72114, or fax 501-228-8651 no later than 24 September 2010.
