Proposal Requirements Project: The Intent of this scope of work is to provide Electrical work for the Correction of Life Safety deficiencies throughout the Centers Main. Building and Dormitories. Owner: US Department of Labor (DOL) Employment and Training Administration Center Operator : Adams and Associates, Inc Mailings address 1101 Mills RD. Joliet, IL 60433 Proposal Due Date: September 9 th, 2010 by close of business 4:00pm Evaluation Criteria: Cost Experience/Service Quality Payment: The Center will make payment within 30 days of completion. Site Visit Date: August 31 st 2010, @ 11:00 am a) Contractor shall visit the site and familiarize himself with the condition under which the work will be performed. No allowance will be made for any error on the contractors part for failure to take into account any conditions or circumstance existing at the site which might affect the contemplate work. The fact that a bid is submitted will be taken as evidence that the contractor has full knowledge of the extent and character of the contemplate work and any problem to be encountered in the performing same. II. Project Description Scope of Project: This project consists of but not limited to replacement and/or installation of exit signage, battery lights, fire alarm and power shop electrical work. The contractors shall provide labor and materials required to fulfill all requirements listed in the following specific requirements attached deficiencies. III Specific Requirements: The contractor shall provide labor and materials required work associated with the referenced electrical and life safety listed deficiencies. See attachments: Life Safety Deficiencies and Electrical Deficiencies. Warrantees: •a) The contractor is to provide written warranty for the above statements at time of completion of job IV Construction Requirements Center Regulations: Contractor and his employees shall abide by all of the Center's rules and regulations. There shall be no fraternization between contractor personnel and Job Corps Center students or personnel. The Contractor will be required to follow all Center safety and security requirements. Management of Jobsite: The Contractor shall maintain a clean and safe workplace that is secured at the end of each working day. The Contractor shall clear all deliveries with the Center's security personnel. Wage Rates: The bidders shall conform to the latest Davis-Bacon prevailing wage rate decisions for the area. Insurance: The Contractor shall obtain and maintain insurance as follows: Workman's Compensation/Employers Liability insurance, to meet New York statutory requirements Comprehensive General Liability ($1,000,000 per occurrence) Comprehensive Automobile Liability ($1,000,000/combined single limit) Fire Legal Liability ($500,000 per occurrence) The Contractor must cover any deductible costs. Upon contract award, the Contractor must add Joliet Job Corps Center as additional named insured to the policies. Schedule: The Contractor shall submit a schedule for the work. The project shall be completed within 30 days of the award of the contract. Written authorization is required to extend the 30 day completion requirements. Final Acceptance The Contractor shall notify the Center's facility manager when the project is substantially complete, and shall request an inspection. The completion of this inspection shall determine the date for the commencement of the warrantees, and the date for payment. V. Proposal Form The Contractor shall submit the following information with the proposal: •1. A list of materials and product data sheets proposed for use. •2. References •3. Copies of Insurance Certificates •4. Completion of Vendors Packet
