Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This requirement is being issued under the Simplified Acquisition Procedures as a Request for Quotation (RFQ) VA-255-10-RQ-0536. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. The government contemplates a firm fixed price contract from this procurement. This procurement will be an unrestricted acquisition. The solicitation and document incorporates provisions and clauses are those in effect through Federal Acquisition Circular (FAC 2005-35, Effective 14 July 2009). The North American Industry Classification System (NAICS) is 493190 with a size standard of 25 million dollars. All prospective Offerors must be actively registered in the Central Contractor Registration. Offeror may register online at https://www.bpn.gov/CCR/scripts/index.html. The award will be made to the low priced Offeror deemed Technically acceptable. The quotation shall consist of two (2) CLINS, Contract Lines for the Base Year and (1) CLIN for Option Years. (See Scope of Work) JOB DESCRIPTION: The contractor shall provide all material and labor necessary to perform Records Retirement, Long term Records Storage, Scanning and Indexing as Outlined in the Scope of Work. The intent of this contract is to procure Medical and Administrative Record retirement, storage, delivery, scanning and indexing services for the John J. Pershing VAMC, 1500 N. Westwood Blvd., Poplar Bluff, MO 63901. (See Scope of Work). CONTRACT OBJECTIVE: The objective of this contract is to procure three (3) distinct records management services. Preparation, transfer and retirement of approximately 13,500 inactive* medical records to the Veterans Affairs Records Center and Vault (VA RC& V), Neosho, MO. Preparation, transfer and storage of approximately 22,500 active records to a NARA approved record storage facility. Retrieval, delivery, and re-file services for approximately 22,500 active records. Professional and experienced medical record document scanning, and indexing on-site services for approximately 400,000 medical record loose documents. *Inactive Medical Records are defined as a record that has not been accessed for three years. (See Scope of Work). SCOPE: Contractor will prepare approximately 36,000 medical records for off-site storage. This will include repair(s) to medical record folders, securing loose documents within the folders, and scanning of miscellaneous documents into the VA VISTA imaging system. Once preparation is complete, records will be barcode scanned into the VA VISTA record tracking system to the appropriate location and then boxed for shipping. Records with missing or unreadable labels will require creation of new labels using the VA VISTA record tracking system options. In addition, a spread sheet must be created with the following information: Patient name; full social security number; record volume number (if more than 1); storage location; and storage box number. All records will be packed in a NARA approved storage box (1.2 Cu FT) following NARA approved packing and labeling guidance. Complete NARA guidance can be found in 36 CFR 1228 subpart K “ box storage (See Scope of Work). SPECIAL INSTRUCTIONS: Contractor Check-in “ The contractors representative will schedule all work with Cathee Helton Poplar Bluff Missouri Veterans Hospital at (573) 778-4190, prior to performance of services. The Contractor shall report all issues to the Contracting Officer ™s Technical Representative (COTR) Cathee Helton while performing work at the Poplar Bluff Missouri Veterans Hospital. Offerors that fail to furnish required representations or technical information as required by FAR Clause 52.212-1 cited below or reject the terms and conditions of this solicitation may be excluded from consideration. The following FAR Clauses and Provisions are incorporated by reference and apply to this acquisition. FAR provision 52.212-1 Instructions to Offerors of Commercial Items (Sep 2006). FAR provision 52.212-3 Offeror Representations and Certifications Commercial Item (Nov 2006). Note: In order to complete the Representations and Certifications for the referenced provisions you must go to the Air Force Web Site at http://farsite.hill/af.mil/Vffar1.htm locate the referenced provision, copy and paste it to a Word Document and complete. If you are registered in the Online Representations and Certifications Application (ORCA) website at https://orca.bpn.gov/ and have the information posted you are only required to submit a signed copy of section (k) of FAR Clause 52.212-3 in place of FAR 52.212-3. FAR 52.212-4 Contract Terms and Conditions Commercial Items (Feb 2007) and FAR 52.247-34 FOB Destination (Nov 1991). FAR 52.212-5 Contract Terms and Conditions required to Implement Statues of Executive Order Commercial Items (Nov 2006) apply to this acquisition. Vendors shall provide three (3) references with contact numbers and a technical plan to complete the records Retirement, long-term records Storage, Scanning and Indexing per the Scope of Work. 52.237-1 This solicitation is being issued as a Request for Quotation (RFQ). Offers shall be clearly marked with RFQ referencing number # VA-255-10-RQ-0536 and mailed to the Point of Contact Listed below no later than then 10 September 2010 at 1:00 p.m. Central Standard Time (CST).
