- The Veterans Affairs Medical Center, Bedford, Massachusetts (Edith Nourse Rogers Memorial Veterans Hospital) has a requirement for solutions to the early diagnosis of Alzheimer disease, mild Traumatic Brain Injury (mBTI) and other neurodegenerative diseases by using non-invasive near-infrared (NIR) spectroscopy in vivo to characterize the light scattering signature of dense protein aggregates. A Request for Proposals (RFP) will be issued to fulfill the requirement. The RFP will be unrestricted. Description: The molecular bases of Alzheimer disease and related neurodegenerative disorders are becoming better understood, but the means for definitive diagnosis and monitoring in vivo remain lacking. Near-infrared optical spectroscopy offers a potential solution. Transmission and reflectance spectra of thin brain tissue slabs have been acquired, from which were calculated wavelength-dependent absorption and reduced scattering coefficients from 470-1000 nm. The reduced scattering coefficients in the near infrared clearly differentiates Alzheimer from control specimens. Diffuse reflectance spectra of gross brain tissue in vitro confirms this observation. These results suggest a means for diagnosing and monitoring Alzheimer disease in vivo, using near-infrared optical spectroscopy. Requirements: The contractor shall design, fabricate and test near-infrared (NIR) optical spectroscopy technology for the non-invasive detection, diagnostic evaluation and monitoring of neurological disorders such as Alzheimer disease and related neurodegenerative disorders (AD), including mild traumatic brain injury (mTBI). The design, fabrication and testing shall be carried out in collaboration with and according to specifications developed by the US Department of Veterans Affairs. The technology provided shall be comprised of three distinct but mutually integral modalities/tasks: (1) hardware, a complete NIR spectrometer system for non-invasively acquiring near-infrared optical spectroscopic measurements, such as absorbance, fluorescence, reflectance, etc. of the human brain in vivo in a clinical setting. The contractor shall provide a multi-channel near-infrared (NIR) optical spectrometer system, which is compact, portable and suitable for clinical or bedside operations. (2) software, source code and executable files for (a) automated spectroscopic data acquisition, monitoring and control and (b) diagnostic algorithms based on spectroscopic data analysis; (3) testing of integrated technology (hardware and software), for safety, performance and specification compliance. In addition, the contractor shall provide Other Technical Support. In collaboration with Bedford VAMC, the contractor shall conduct both laboratory testing in house and at the Bedford, Massachusetts VA Medical Center (Bedford VAMC) as well as clinical testing at Bedford VAMC and/or participating clinical sites. The period of performance is fifteen (15) months from contract award. The Government reserves the right to make a single or multiple award. A firm-fixed price contract is under consideration. The Government reserves the right to award with discussions. The estimated Level of Effort (LOE) is approximately 1,400 to 1,600 hours. The NAICS is 541712. The PSC is AN12.
