- Attachement 1 Attachment 2 This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Federal Acquisition Requirements (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued. The solicitation number for this requirement is 2010-Q-86657 and is hereby issued as a Request for Quote (RFQ) using FAR Subpart 13.1 Simplified Acquisition Procedures. The Centers for Disease Control and Prevention intends to award a firm-fixed-price contract for the following requirement, please either fill in the dollar amounts or provide a separate excel spreadsheet containing the same information: CLIN 0001: Develop a plan for the content and logistics of the workshop. $ CLIN 0002: Reserve location, develop materials, publicize event and coordinate registration. $ CLIN 0003: Abstract submission and review process. $ CLIN 0004: Host the Workshop. $ CLIN 0005: Prepare manuscripts suitable for publication. $ (All work shall be performed in accordance with the attached Statement of Work (SOW)) Your quote shall include the following: • Price for each CLINs (1 - 5) and Grand Total to complete Job. Cost - For evaluation purposes, costs should be clearly broken down for each unit, to include Parts, Labor other expenses. • Capabilities in accordance with the Statement of Work and Evaluation Factors This acquisition is under North American Industry Classification System code 541990. It is the offeror's responsibility to be familiar with the applicable clauses and provisions. Clauses and provisions may be accessed via the Internet at website http://farsite.hill.af.mil. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-43.
