- RADIOLOGY-NUCLEAR MEDICINE RADIOLOGICAL SURVEY - PHYSICS CONTRACT Radiation Safety Evaluation (Survey) This Sources Sought Notice is for informational and planning purposes only and shall not be construed as a solicitation or as an obligation on the part of the Department of Veterans Affairs. The purpose of this Sources Sought notice is to identify technically qualified businesses than can provide a certified health physicist for radiology, dental and nuclear medicine physics testing. The service shall require quarterly physicist testing and annual Radiation Safety Evaluation (survey). The physicist shall perform inspections, procedures and testing and Annual radiological physics surveys as required by Joint Commission (JC), the Americal College of Radiology (ACR), Food & Drug Administration (FDA) and Occupational Safety & Health Administration (OSHA) that meet or exceed current standards established by JC, ACR, FDA, OSHA. Services shall be performed at the Orlando VA Health System at the following locations: 1. Orlando VA Medical Center, 5201 Raymond Street, Orlando, FL, 32803 2. Viera VA Outpatient Clinic, 2900 Veterans Way, Viera, FL 32940 3. William V. Chappell, Jr. VA Outpatient Clinic, 551 National Health Care Drive, Daytona Beach, FL 32114 Responses to this notice should include the following: Name of company/provider, business size (large or small); identify if the company/provider falls into any of the following socioeconomic categories: Service Disabled veteran owned small businesses; Veteran owned small businesses, 8(a) small businesses, women-owned small businesses, HUBZone small businesses, or Small disadvantaged businesses, and the company/provider's size classification relative the North American Industry Classification System (NAICS) code. Please include information on professional health physicist services for radiological inspections / Medical Physics Services previously performed at any VA medical centers and/or other government entities, to include contract numbers, dates of performance and points of contact. Include any other pertinent information which demonstrates the company/provider's ability to meet the above requirement. This is NOT a solicitation for proposals, proposal abstracts, or quotations; no formal solicitation documentation exists at this time. Responses to the informationn requested will assist the Government in determining the appropriate acquisition method, including if a set-aside is possible. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. After a review of the responses received, a pre-solicitation synopsis/solicitation may be published in Federal Business Opportunities. However, responses to this notice will not be considered adequate responses to a solicitation. Confidentiality: No proprietary, classified, confidential, or sensitive information should be included in your response. The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s).
