Description
Psychiatrist Services Portland VA Medical Center This is a combined synopsis/solicitation for the purchase of commercial items, prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the Quotes are being requested and a written solicitation will not be issued. Solicitation number VA-260-10-RQ-1162 is applicable and is issued as a request for quotations. The solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-44. This procurement is a 100% small business set-aside only solicitation. Quotes are being requested and a written solicitation will not be issued. The relevant NAICS Code is 621112 with a corresponding size standard of 100 employees. This is NOT a locum tenens solicitation. Once the Contracting Officer has decided to whom an order will be issued, a purchase order will be issued using a SF 1449. The description of the commercial services is as follows: The Portland (Oregon) U.S. Veterans Administration Medical Center (PVAMC) is seeking minimal, part-time Psychiatrist to provide support services to the area Veterans Center. This is NOT a locum tenens solicitation. The Portland Vet Center needs 4 hours and the Salem Vet Center needs 3 hours of clinical staff support services per month. The Psychiatrist will provide information on psychiatric medications that are used in dealing with PTSD treatment, side effects and alternative treatment. The individual will also conduct case reviews of complex diagnostic or treatment interventions and provide feedback to counselors delivering care to veterans, as well as, monitor ongoing quality of care process to ensure veterans are receiving the most efficient and essential care available in the community. Contractor must be a licensed of the state of Oregon and is a US citizen. The Psychiatrist will be required to pass background investigations, including verification of identity, education, licensure and professional credentials. The provision at FAR 52.212-1, Instructions to Offerors Commercial (JAN 2004), applies to this acquisition. In accordance with 52.212-3, Representations and Certifications, offers must either be accompanied by Representations and Certifications or the offeror must be currently registered in ORCA at website https://orca.bpn.gov/. The clause at FAR 52.212-4, Contract Terms and Conditions -- Commercial Items (OCT 2003) applies to this acquisition. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders --Commercial Items (Jul 2010) applies, The following clauses within FAR clause 52.212-5 apply: 52.217-8, 52.217-9, 52.219-28, 52.222-3, 52.222-19, 52.222-41, 52.222-41,52.222-36, 52.232-18 and 52.232-34. VAAR clause 852.237-7 Medical Liability Insurance, 852.273-75, 852.203-70 applies. The closing date for receipt of quotes is August 30, 2010 at 3:00 PM Pacific Daylight Time. Vendors offering a response to this notice should ensure registration in the Central Contractor Registration database at http://www.bpn.gov/ and in the Vet Biz registry at http://www.vip.vetbiz.gov/ prior to submission of offers for consideration.
