- This combined synopsis/solicitation shall constitute the only announcement for this requirement. Quotations are being requested and written solicitation will not be issued. A Concessionaire Contract is contemplated. Solicitation Number FA4861-10-R-B030. 1. Nellis AFB is requesting quotations form qualified businesses interested in providing the Barber Shop Concession service at the O' Club, Nellis AFB, Nevada for period of ONE (1) YEAR, beginning 1 Oct 2010 and ending 30 Sep 2011. The contract will include four (4) option years for a total contract lifespan of five (5) years. 2. If you wish to compete for the opportunity to provide this service, please submit a package not later than 25 Aug 2010, 12:00 noon. Please note that the successful contractor must comply with all elements of the Performance Work Statement (PWS). (Attachment 1) 3. Your package must include the following, and all items must be completed and provided to be considered fully responsive/eligible for award: a. A cover sheet with the solicitation number FA4861-01-R-B030, your name, the company name, address, phone number, fax number, e-mail address, and DUNS number. b. A completed Solicitation to include Article I-IV, filling in the blanks on page 3, section 1.2.6.1 (Attachment 1). Note that the Non-Appropriated Fund (NAF) Special Provisions, along with the completed Articles, comprise the basic Concessionaire contract document. c. A completed Description of Services, (i.e. a list of the prices your company will charge for services provided) Note: Only the services on the Description are solicited or required at this time. In accordance with the PWS, expanded services may be offered after contract award. These prices will not be considered for basis of evaluation and award. d. A copy of current State license/certification card. 4. All quotes must be sent via email to Darcella.fox.ctr@nellis.af.mil, phone 702-652-8402. 5. An award will be made to responsive, responsible contractor who is determined to offer the best value to the government based on the highest percentage of gross sales to pay the Non-Appropriated Fund Instrumentality (NAFI). 6. The Solicitation, which will become the contract after annotation by the successful offerors, shall include Articles I-IV, Special Provisions and NAF General Provisions. Attachment: 1.
