- Statement of Work for Medical Courier Services This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested for request for quote. Solicitation number F1U4M20167A001 is hereby issued as a Request for Quote (RFQ). The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-32. This acquisition is 100% set aside for small business concerns and is under North American Industry Classification Standards (NAICS) code 492110, with number of employees not to exceed 1,500. DESCRIPTION OF REQUIREMENT: The contractor shall provide all management, tools, supplies, equipment, and labor necessary to furnish the following to Eielson AFB, AK in accordance with attached Statement of Work. 0001 - Courier service to deliver medical records and laboratory samples to Bassett Army Community Hospital once per day between 0730 and 1400 M-F except Federal Holidays or Down Days (12 Months; 21 Sep 10 - 20 Sep 11). 0002 - Dry ice pick-up from Arctic Fire and Safety in Fairbanks, AK and deliver to 354 MDG Laboratory each Monday (52 Weeks) before 1600. Weight will vary dependent up season. Currently 20 lbs during the summer and 10 lbs during the winter. F.O.B. DESTINATION IS TO EIELSON AFB, AK (End description). Quotes must be submitted and received no later than 8 September 2010 at 4:30pm Alaska Daylight Time. Quotes may be e-mailed or faxed to 907-377-2389 to the attention of primary P.O.C.s listed below.
