Description
SOURCES SOUGHT INJURY PREVENTION AND HUMAN PERFORMANCE OPTIMIZATION SERVICES This is a Sources Sought Notice (not a presolicitation notice pursuant to FAR Part 5) to identify HUBZone certified sources that can provide certified athletic trainers (ATs) and certified strengthening and conditioning services (CSCS) at four (4) predetermined locations in Missouri, Georgia, Oklahoma and Virginia. The number of ATs and CSCS required at each location will vary dependent upon the number of battalions assigned. A battalion can range from 300-1200 individuals. Currently in Missouri five (5) battalions can be expected; Georgia, four (4) battalions; Oklahoma, five (5) battalions and Virginia two (2) battalions. Services will include all of the following and are expounded upon in the performance work statement: (1) identify through observation or examination, Soldiers who display signs or symptoms of musculoskeletal or heat injuries; (2) provide treatment to Soldiers who experience an acute musculoskeletal or heat injury; (3) follow rehabilitative courses of treatment that have been developed by Army physical therapists or physicians; (4) develop and implement rehabilitative courses of treatment for Soldiers who have musculoskeletal injury; (5) collect and provide required/designated monthly data regarding injury prevention and injury surveillance and provide monthly reports regarding all work activities performed; (6) mitigate or prevent musculoskeletal and heat injuries, if possible; (7) develop, implement and facilitate individual and group remedial exercise programs for Soldiers with musculoskeletal injuries and/or physical restrictions; (8) observe physical readiness training and other training events for the purpose of providing injury prevention and human performance optimization guidance; and (9) provide input to subject matter expert in areas of injury prevention, injury treatment, remedial training and rehabilitative training. Services rendered shall conform to the highest standards in accordance with applicable Army and Department of Defense publications, and the attached DRAFT performance work statement (PWS). This survey is to determine the capability of potential HUBZone certified sources and the availability of commercial sources and services pursuant to FAR Part 10. This notice is for informational purposes only and does not constitute an Invitation for Bid nor Request for Proposal, and is not to be construed as a commitment by the Government. The Government will not reimburse contractors for any cost incurred for their participation in this survey. Responses should contain Company Name, Address, Point of Contact, Cage Code, Phone Number, FAX Number, Size of Business pursuant to NAICS code 621340 (size standard $7,000,000), a brief resume of performance of similar services (since January 2007) of the magnitude described above (include contact and phone number of projects listed), a statement as to whether your company is domestic or foreign owned. In addition to the size of business, indicated all classes of business(es) that fit your firm; i.e. HUBZone, Small Disadvantaged, Veteran-Owned, Woman-Owned, etc. Responders should also indicate the approximate time it would take to hire/staff the tasks/locations listed above.
