- Clauses/Provisions Statement of Work RFQ This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, in conjunction with FAR 13.5, as applicable, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is 21-10-340482A45 and is issued as a Request for Quote (RFQ), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-44. The associated North American Industrial Classification System (NAICS) code for this procurement is 334310 with a small business size standard of 750. This requirement is a Small Business set-aside and only qualified offerors may submit bids. The selected Offeror must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, paragraph (a) and the following clauses in paragraph (b): 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, 52.232-34. All Contractors doing business with the Government must be registered with the Central Contractor Registry (CCR). No award can be made to a company not registered in CCR. For additional information and to register in CCR, please access the following website: http://www.ccr.gov. In order to register all firms must have a Dun and Bradstreet Number. A Dun and Bradstreet number may be obtained free of charge by contacting Dun and Bradstreet at http://www.dnb.com/us/duns_update/ or 800.234.3867. Offerors are encouraged to go on-line at www.bpn.gov to complete the representations and certifications (ORCA). The U. S. Coast Guard, Honolulu, HI has a requirement for the services and/or supplies as listed below. Please fax quotation and completed 52.212-3 Offeror Representations and Certifications-Commercial Items form to BSU Honolulu (pc), Attn: Nadyne Cano at(808)842-2834 or email to Nadyne.V.Cano@uscg.mil as soon as possible, but no later than 12:00 p.m. HST, August 26, 2010. The contractor shall provide the following equipment as listed below. Contractor shall also provide labor, equipment, materials, tools, parts, and all other items necessary for installation services PER ATTACHED STATEMENT OF WORK. 1. 70" TOUCH SCREEN LCD display including wall mounting hardware 2. Four (4) 42" LCD displays including wall mounting and hardware 3. Audio/Video matrix switch with minimum of eight (8) inputs and eight (8) outputs with serial control and stereo audio 4. DVD player/recorder with a minimum of 320 GB hard drive. 5. 1200-Watt Stereo Power amplifier/mixer with surround sound capability. 6. Minimum 2 channel audio/video pre-amplifier with four (4) audio inputs, four (4) composite video inputs and RS-232 control. 7. Audio speakers to support surround sound within the conference room. 8. Computer to host control software for audiovisual system. 9. Connection for the video/output of standalone laptop into the audiovisual system. 10. Any required network gear to connect individual components together 11. Server rack with wiring and power distributions systems along with required shelving. At minimum, the control computer, audio/video matrix, pre-amplifier, amplifier/mixer, DVD player and required network gear shall be installed in the server rack. All equipment shall be connected to a UPS. 12. All required cabling and connectors required for the install. Proper junction boxes, telecommunications outlet boxes, pass-through wall plates. 13. UPS SEE STATEMENT OF WORK FOR ADDITIONAL REQUIREMENTS; SPECIFICATIONS; INSTRUCTIONS AND/OR INFORMATION. Site visit prior to submission of proposal is highly encouraged.
