- This is a combined synopsis/solicitation prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested. The following provisions and clauses apply plus any addenda: FAR 52.212-1 Instructions to Offerors – Commercial item; 52,212-2 Evaluation – Commercial Items; 52.212-3 Offeror Representations and Certifications – Commercial Item; 52.212-4 Contract Terms and Conditions – Commercial Item; and 52.212-5 Contract terms and Conditions to Implement Statutes or Executive Orders – Commercial Items. NAICS for this requirement is 339112. The U.S. Naval Hospital, Okinawa Japan has a requirement for CLIN 0001: (mftr #V500-BCDT) Integrity V500 System. Vivosonic Inc., UNIT PRICE, QUANTITY: 1. CLIN 0002: Training for the System to Okinawa Japan UNIT PRICE, QUANTITY: 1. CLIN 0003: Shipping and Handling UNIT PRICE, QUANTITY: 1. TOTAL PRICE, SPECS: For Auditory diagnostics in infants through seniors. Indicate for auditory evoked response testing as an aid in detecting hearing loss and lesions in the auditory pathway. Prescription device. Portable PC-based, consisting of hardware and computer software, and configured in a carrying case. Software Specifications: Modules: Auditory Brainstem Response (diagnostic and threshold), Automated Screening ABR, Electrocochleography, Distortion Product Otoacoustic Emission, Transient Evoked Otoacoustic Emission, Auditory Steady State Response. Dimensions: H1.2” x L18cm x W9.1 cm, Weight 350g. Electrical Data: Voltage 110V, Frequency: 50Hz. The Contractor shall responsible for delivery to our FPO, AP address. Providing proof of delivery and tracking information (tracking number) shall be provided to the Government upon request. Contractor shall re-ship orders, at their expense, if proof of deliver and proper tracking information cannot be provided. This notice of intent is nor a request for competitive quotes; how ever, any firm that believes it can meet the requirements may give written notification to the Contracting Officer within 10 calendar days Aug 27, 11:00 AM Japan Standard Time: from the date of publication of this synopsis. Supporting evidence must be furnished in sufficient detail, to demonstrate the contractor’s capability to provide any ability to comply with the above requirements. Information can be e-mailed to the Contracting Officer. The Government will award a contract resulting from this solicitation to the responsible Contractors whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotes: (1) Technical Capability of the item offered to meet the Government requirement (including submission of descriptive literature); (2) Price; all to be submitted with quotation. No telephone requests will be accepted. Responses received will be evaluated, however, a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. Faxed quotes will be accepted. Contractors are responsible for obtaining any and all amendments or additional information.
