Attachment 4 - RFQ Information Sheet as amended 16 Aug 10 Attachment 3 - First Article Test as amended 16 Aug 10 Attachment 2 - Shipping Locations Attachment 1 - Essential Characteristics as amended 16 Aug 10 This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This acquisition is set-aside for small business IAW FAR 19. Therefore, only quotes submitted from small businesses in response to this RFQ will be considered for award. The North American Industry Classification System (NAICS) for this acquisition is 339112 with a Small Business Size Standard of 500 employees. Please identify your business size in your response based upon this standard. The solicitation number for this announcement is Request for Quote (RFQ) FA4452-10-Q-A038. Only one contract award will be made to one contractor offering the lowest total priced technically acceptable quote meeting the essential characteristics. Requirement is to provide a quantity of twenty (20) Medical Air Compressor Devices as specified in Attachment 1: Essential Characteristics. The items will need to be delivered to 7 CONUS and 4 OCONUS locations (see Attachment 2: Shipping Locations). The lowest priced technically acceptable solution will need to undergo First Article Testing IAW FAR 9.3 in order to obtain joint air worthiness certification (see Attachment 3: First Article Testing for more details). Please note that the Government will reimburse the fee for obtaining proof of compliance from the Food and Drug Administration on a cost basis. A response to this request shall consist of completion of Attachment 4, Request for Quote FA4452-10-Q-A038 Information Sheet and submission of descriptive literature that addresses the requirements listed within this synopsis/solicitation. Quotes are to be submitted FOB Destination. Price quotes shall include any shipping cost within the unit price. Required delivery for the First Article Test unit is NLT 180 days after date of contract award (ADC). Required delivery for remaining units is NLT 60 days after First Article Test Approval (AFATA). The Government reserves the right to cancel this solicitation, with no obligation to the offeror by the Government. AFFARS 5352.201-9101 Ombudsman - (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of quotes, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings.
