Description
REQUEST FOR PROPOSAL Instructional Certified Medical Trader The South Bronx Job Corps Academy is a Federally Funded Vocational and Educational Training Program that services youth between the ages of 16 and 24 in a residential setting. The program operates year round with approximately 200 residential and 75 non-residential trainees. The program is operated by ResCare, Inc. for the United States Department of Labor. The South Bronx Job Corps Academy is soliciting Bids for Instructional Certified Medical Trader programs. All interested bidders need to develop and submit a bid in accordance to the scope of work listed below, including specifications and a fully broken down cost fees. SCOPE OF WORK Certified Nurse Aide - up to 24 Pharmacy Technician Medical Assistant- up to 24 students EKG/ Phlebotomy tech & Patient Care tech-up to 30 students This Agreement is entered into upon the following terms and conditions: 1. The Training center is will provide the services set forth in the scope of work and incorporated herein, and agrees perform said services and to provide all necessary staff support and administrative services connected therewith. 2. In accordance with the SBJCA accountability process, The Training Center shall fax student attendance on a daily basis no later than 10AM. 4. The training center class schedule will be M-F from 9am-4pm. The training center must incorporate hands on skills that are required by the program. The training center must provide internship /externship for up to six (6) weeks The Training center must be able to provide the students with additional help such as; afterschool and/or weekend make up class or tutoring in the program. 5. The Training Center will provide the recommended textbooks; materials, uniforms and supplies to course participants. 6. The training center must provide placement assistant for graduate for the program 7. This Agreement must be governed by the laws of the State of New York except where the Federal supremacy clause requires otherwise. Disputes involving this Agreement, including the breach or alleged breach thereof, may not be submitted to binding arbitration (except where statutorily authorized) but shall, instead, be heard in a court of competent jurisdiction of the State of New York. REQUIREMENTS •· Must be in compliance with all Local, State and Federal Guidelines and Regulations •· Work to be accomplished in safe and efficient manner with minimal or no disruption or endangerment to trainees and / or staff. •· Must provide a copy of current licensure and insurance LENGTH OF AGREEMENT Bidders must provide time frame in which work is to be completed. This Contract is for one year. ACKNOWLEDGEMENT OF ACCEPTANCE •· Contractor must acknowledge by signature that they meet all Federal, State, Local and OSHA regulations and that all specifications terms and conditions set forth are acceptable. •· Bidders must submit their proposal in a sealed envelope by 5 p.m. close of business August 25, 2010 to the attention of Vivian Lopez, Purchasing Agent. The following documents Davis Bacon Act (40 U 27a Attachment B), Certification of Non-Segregated Facilities Pursuant to FAR 52.222-21 (Attachment C), Equal Opportunity and Affirmative Action Requirement Pursuant to FAR 52.222-26 and FAR 52.222-27, FAR 52.252-2, 52.204-1, 52.215-19, PRC 4121 and 4170 will be incorporated and made part of agreement.
