Description
INQUIRIES AND RESPONSES TO THE ABOVE RFQ. RESPONDED AND POSTED THIS DATE- 8/13/10. THIS DOES NOT ALTER THE RESPONSE DUE DATE. Solicitation Number: SSA-RFQ-R03-10-1016 "The successful vendor shall provide a maximum of 700 Influenza inoculations on site at the Social Security Administration, WBDOC, 1150 East Mountain Drive, Wilkes Barre PA 18702. The number of Influenza Inoculations are an estimate only, but shall not exceed the 700 identified." Question #1: Is it mandatory that all Social Security Administration personnel at this location be required to receive but not to exceed 700 doses of the Influenza Inoculations ? Answer #1: It is not mandatory for Social Security Administration personnel to receive inoculations. The 700 doses of inoculations are an estimate only. It may be less than the 700, but will not exceed 700. Further, the vendor will only be paid for the number of inoculations provided. Question #2: Will the Social Security Administration provide to the Contractor a pre-survey of / or pre-register of it's personnel as to the exact number of personnel accepting the Influenza Inoculations and before Vaccine supply is ordered as to prevent purchase of excess doses and or waste of materials and supplies in an effort to control inventory needs and or monetary lost. Answer #2: See above for a partial answer. We have a list of individuals, however, they may accept or decline to receive the inoculations. It is their choice. The 700 figure is only an estimate. "This is a multi shift facility and therefore multiple clinics shall be required to fulfill the requirement." Question #3: What are the days and start and end time of operations of the first shift? Answer #3: That is unknown to this office. The operation is 24/7. No clinic will be expected to beyond 5 PM. Question #4: What are the days and start and end time of operations of the second shift? Answer #4: As above Question #5: What are the days and start and end time of operations of the third shift? Answer #5: As above Question #6: Are their any other configurations or odd shifts days and start and end times? Answer #6: This is unknown to this office. Please see the above answers. This best describes the operation. Question #7: Will the Social Security Administration at this site provide a room for the Vaccine Clinic with washroom / sink facility and refrigerator for safe Vaccine handling and storage? Answer #7: The site has a operational medical suite. The vendor will not responsible for the storage of the vaccine. SSA cannot be responsible for the vendor's product. Question #8: Has there been other Vaccine Clinics operated from this location in the past year(s)? Answer #8: Yes. "Payment shall be made by Government Purchase Card for only the number of influenza inoculations provided to the facility." Question #9: How shall the Government Purchase Card transaction(s) proceed? Example: A transaction per influenza inoculations, a transaction per lump sum total number of inoculations that day, a transaction per lump sum total number of inoculations that week or a transaction per lump sum at the completion of influenza inoculations clinic. Answer #9: The successful vendor shall submit a proper invoice, which includes: •· the description of the item/service provided, •· the cost per inoculation, extended price •· Duns and Tax Identification Number •· Purchase Order Number •· Full name and address of the vendor In addition to the above, the vendor shall provide a list of names and dates of the individuals inoculated. This will be compared against the list provided by the SSA on site medical staff. Question #10: Will the Government Purchase Card be limited to not to exceed $2,000.00 per transaction as has some practice in the past or could this be waived by conditions? Answer #10: I am a Contracting Officer and my purchase card does not have this limitation. As a point of reference, there are several levels of limitations affixed to Government purchase cards. The $2000 limit is the micro-purchase limit for construction. AS A REMINDER- THIS IS AN UNRESTRICTED ACQUISITION.
