- This is a combined synopsis/solicitation for commercial item prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This request for quote (RFQ) N00174-10-Q-0188 is the only written solicitation that will be issued, unless amended. The solicitation document, incorporated provisions and clauses are those in effect through Federal Acquisition Circular 18. The NAICS code 334517. Size standard 500 employees. NSWC Indian Head has a requirement for one Brand name mandatory DyanmIx HR/Series 5 system. The offeror shall provide a firm fixed price offer for Quantity of one DynamIx HR/Series 5 System with the following characteristics: DynamIx HR/Series 5 with brilliant five mega pixel flat panel display, reader unit, and input terminal; imaging plate UR-1 size 14 x 17 , quantity 10; imaging plate UR-1 size 10 x 12 , quantity 10; imaging plate UR-1 size 7 x 9.6 , quantity 10; cassette CR magazine type size 14 x 17 , quantity 4; cassette read feed type size 10 x 12 , quantity 2; cassette read feed type size 7 x 9.6 , quantity 2; cassette CR magazine type with lead (0.010 thickness), quantity 4; calibration kit for brilliant five display, quantity 1; cart for system, quantity 1; DynamIx Image Share œEX, quantity 1; DICOM Output Key, quantity 1; Image Additional Key, quantity 1; touch screen flat panel display for reader input terminal; lifetime software licensing; one year warranty, including parts and labor; training “ up to sixteen hours. DELIVERY FOB destination, 45 days after effective date of order. The following provisions apply: 52.212-1, Instructions to Offerors-Commercial; 52.212-4, Contract Terms and Conditions-Commercial Items, Addendum to 52.212-4 DFARS 252.211-7003 Item Identifier and Valuation; 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders; Commercial Items 52.212-2, Evaluation-Commercial Items - the award will be made to the lowest priced, technically acceptable offeror. Offerors shall provide a copy of the complete technical specifications. Offeror shall affirmatively address their ability to meet each of the specifications individually and address the ability to meet the desired delivery requirement. NO PRICING IS TO BE PROVIDED IN THE TECHNICAL INFORMATION. Offeror shall provide pricing, in a separate volume, to include any warranty, installation and delivery to destination. Failure to address any of the instructions within this combined synopsis/solicitation may result in a proposal being considered unacceptable. Intent is to award based on initial offers. If considered necessary by the contracting officer, discussions will be conducted only with those offerors determined to have a reasonable chance for award. Offerors must provide with their proposal a completed copy of FAR 52.212-3, Offeror Representations and Certifications-Commercial Items (June 2003) or ORCA showing above referenced NAICS code. Responses must be received no later than 3:00 p.m. EST on 30 August 2010.
