- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This combined synopsis/solicitation is issued as a request for quote (RFQ) # Creek Environmental Analysis Submit written offers. Oral offers will not be accepted. All firms or individuals responding must be registered with the Central Contractor Registration (CCR) (www.ccr.gov). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-43 and DFARS Change Notice DCN 20100713. North American Industrial Classification Standard (NAICS) 541380 and Size Standard $12.0 million apply to this procurement. Project Number: Creek Environmental Analysis Project Title: Creek Environmental Analysis This is for a BLANKET PURCHASE AGREEMENT (BPA). Services, non-personal: Contractor to furnish all labor, parts, tools, transportation, materials, equipment, facilities, and supervision necessary to provide Management of the Creek Environmental Analysis as stated in the Statement of Work: POP: 16 August 2010- 15 August 2015 The following Clauses apply to this award: 52.204-9 Personal Identity Verification of Contractor Personnel. 52.212-1 Instructions to Offerors- Commercial Items. ADDENDUM: 1. Contractor shall provide evidence of lab certifications (ELAP) 2. Contractor shall list methods used for analytes required by Vandenberg 3. Contractor shall provide past performance history within the past 5 years 4. Contractor shall provide Quality Assurance procedures. 5. All questions must be submitted in writing NLT 8:00am August 10, 2010 to Jessica Nelson, jessica.nelson@vandenberg.af.mil 52.212-2 Evaluation-Commercial Items. Offers will be Evaluated based on Technical Capabilities, Past Performance, Price, and Best Value. ADDENDUM: 1. The proposal shall be evaluated based on technical capabilities, past performance, price, and best value. Technical Capabilities: The contractor shall provide a capabilities statement explaining the methods they intend to use for analytes, as well as explaining their quality assurance procedures. The contractor shall provide evidence of lab certifications. Past Performance: Contractor shall provide evidence of past performance history. Past performance must be on similar projects performed within the past five years, the contractor's ability to provide water analysis. Price: The Contractor will be evaluated based on overall price of providing these services. Best Value: In the event that there are multiple contractors with similar attributes, the awarding will be based on best value of the aforementioned criteria.
