Description
Performance Work Statement This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number FA8900-10-R-0201 is issued as a request for proposal (RFP). The applicable NAICS code is 541990, All Other Professional, Scientific, and Technical Services. The attached Performance Work Statement (PWS) sets forth the requirements for an Infectious Disease Specialist to assist with developing, producing, and teaching exportable courses in the areas of Pandemic Influenza, HIV, Ebola, Malaria and Tuberculosis to students representing foreign governments and international health agencies. Other services include organizing bio-security conferences that will be attended by senior military and civilian foreign government officials, and leading/participating in other bio-security related subject matter exchanges. Period of Performance (POP) will not exceed 12 months after date of award. Services will be performed at The USAF School of Aerospace Medicine (USAFSAM), Defense Institute for Medical Operations (DIMO), Brooks City Base TX. Offerors must include with their quotations: a completed copy of provisions 52.212-3, Offeror Representations and Certifications--Commercial Items, obtained from Air Force Far Site, http://farsite.hill.af.mil. The following provisions and clauses can be viewed through Internet access at Air Force FAR Site, http://farsite.hill.af.mil. The following clauses and/or provisions apply to this solicitation: FAR 52.212-1, Instructions to Offerors Commercial Items, FAR 52.212-2 Evaluation Commercial Items. The following factors shall be used to evaluate offers: (1) Technical Capability: Board certified and practicing infectious disease specialist with minimum 10 years experience; Minimum 10 experience with Military operations; International education and training with minimum of 5 years experience; SECRET Security Clearance; (2) Price, and (3) Past performance. Contractors are required to be Registered in the Central Contractors Registration (CCR). AF 5352.201-9101 OMBUDSMAN (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions). (c) If resolution cannot be made by the contracting officer, concerned parties may contact the Center/MAJCOM ombudsmen, Mr Howard E. Concerns, issues, disagreements, and recommendations that cannot be resolved at the MAJCOM/DRU level, may be brought by the concerned party for further consideration to the Air Force ombudsman, Associate Deputy Assistant Secretary (ADAS) (Contracting), SAF/AQC, 1060 Air Force Pentagon, Washington DC 20330-1060, phone number (703) 588-7004, facsimile number (703) 588-1067. (d) The ombudsman has no authority to render a decision that binds the agency. (e) Do not contact the ombudsman to request copies of the solicitation, verify offer due date, or clarify technical requirements. Such inquiries shall be directed to the Contracting Officer. If not already registered, you should immediately register with CCR by calling 1-888-227-2423 or via the Internet at https://www.ccr.dlis.dla.mil/ccr/scripts/index.html. Contractors are required to have a Dun & Bradstreet number (DUNS) in order to be registered in the CCR database. Lack of registration prior to award, may make an offeror ineligible for award. A contractor can receive a DUNS number by calling Dun & Bradstreet at (800) 333-0505. The Defense Priorities and Allocations System (DPAS) is not applicable to this acquisition.
