- Items on this RFQ are Item# 81-60000-000, Neurostar TMS Therapy System (includes console, chair, accessories, and service warranty) & Item# 81-50050-000, 50 Senstar Treatment Links (includes 50 Neurostar TMS Therapy treatment packs) for the Psychiatry Department manufactured by Neuronetics, Inc. PLEASE, NO SUBSTITUTIONS. Quotes should reflect pricing on all items for consideration. Items on this RFQ are Item# 81-60000-000, Neurostar TMS Therapy System (includes console, chair, accessories, and service warranty) & Item# 81-50050-000, 50 Senstar Treatment Links (includes 50 Neurostar TMS Therapy treatment packs) for the Psychiatry Department manufactured by Neuronetics, Inc. PLEASE, NO SUBSTITUTIONS. Quotes should reflect pricing on all items for consideration. All supplied items are to be certified for use in US based hospitals adhering to UL and JACHO requirements as they apply. Please provide government & destination pricing. Also, state warranty information for each CLIN number (Line Item). All items are to be warranted against manufacturer's defects. Vendor must replace all defective items at no additional cost to the Government. Vendor's price to be inclusive of all cost including shipping. Contractor shall supply a current published price list or GSA/VA Federal Supply Schedule contract for supplies &/or services being procured. Refrigerated, frozen, or hazardous material must be indicated as such & a copy of the MSDS (Material Safety Data Sheet) must be supplied for any hazardous material. Specify any special handling instructions. Solicitation closes on Monday, 16 August 2010.
