- This is a Sources Sought notice only and is for information and planning purposes and is not a Solicitation announcement. The purpose of this Sources Sought announcement is intended for Market Research to acquire information of a potential qualified source to Provide Diagnostic Proactive Memory Services related to traumatic brain injury, post-traumatic stress disorder and progressive dementing diseases causing memory disorders to be highly prevalent among veterans. This innovative program will provide brief cognitive assessment, diagnostic memory evaluations, treatment recommendations and proactive management plans and results of qualitative and quantitative outcome and evaluation measures to beneficiaries of the VA Salt Lake City Health Care System in highly rural Western Wyoming, Eastern Idaho and Southern Montana under the VA's Expanded Sharing Authority contained in 38 USC 8153. The Contractor will conduct assessments in Jackson Wyoming, which is located 92 miles from the nearest Community Based Outpatient Clinic (CBOC) in Pocatello Idaho and more than 270 miles and approximately 5 hours driving time from the Salt Lake City, Utah VA, the closest facility currently offering these services. Six (6) Idaho counties and three (3) Wyoming counties closes to Jackson Wyoming are the primary catchment area for this program. The number of enrolled veterans in these counties is 4,291, with 38% of enrollees not currently patients of the VA. The total of non-enrolled veterans is unknown, but likely to be substantial due to the remoteness of current facilities. No veterans in this catchment meet VA access standards. The five CBOCs located closest to Jackson Wyoming are also rural and have limited access. These CBOCs, in addition to Pocatello Idaho, are Riverton Wyoming (124 miles distant), Powell Wyoming (134 miles), Rock Springs Wyoming (152 miles) and Bozeman Montana (153 miles. The contract performance period will be October 1, 2010 through September 30, 2011, with provisions for one 12 month option period. The North American Industry Classification System (NAICS) code for this acquisition is 622110 and size standard $34.5m. All prospective offerors in response to any Federal Agency must obtain a Dun and Bradstreet Number at www.dnb.com. There is no cost to obtain this number. Offerors must also register at the Central Contractor Registration site at www.ccr.gov. Offerors must complete Online Representations and Certifications (ORCA), https://orca.bpn.gov. Additionally, all Service Disabled Veteran Owned Business or Veteran Owned Business must be registered with the Department of Veterans Affairs Center of Veterans Enterprise Vet Biz Registry located at http://vip.vet.gov. All interested concerns must submit their company name, D&B Number, type of business concern and a statement of capability and qualifications to perform the requirements to the Contracting Officer.
