- Please note that this is not a request for quotes or proposals; the Government is seeking information for market research purposes only. Your responses to the information requested will assist the Government in determining the appropriate acquisition method. A solicitation is not currently available. The Government may or may not issue solicitation documents. No formal solicitation document exists at this time. The Department of Veterans Affairs, Veterans Integrated Service Network (VISN 3), James J. Peters VA Medical Center, Bronx New York, Network Acquisitions and Logistics program is currently seeking firms, Service Disabled Veteran Owned Small Business, Veteran Owned Small Business, Hubzone, 8(A) OR Small Business with the ability to provide "AMBULETTE SERVICES" for beneficiaries of the NY Harbor Healthcare Facilities which includes the Bronx VA Medical Center located at 130 West Kingsbridge Rd., Bronx, NY 10468, Manhattan VA located at 423 East 23rd St., NY, NY 10010; Brooklyn VA located at 800 Poly Place, Brooklyn, NY 11209 & St. Albans Medical Facility located at 179 St. & Linden Blvd., St. Albans, NY 11425 and various Community-Based Outpatient Clinics (CBOC's). Vendor will be responsible for providing all vehicles, management, supervision, personnel, materials, supplies, equipment and reports and shall plan, schedule, coordinate and assure effective performance of these required services. Vendor(s) need to respond to this request ONLY if you are capable and well experienced in providing this type of service. Vendor shall have all the certifications, licenses and permits required to provide the service that meet all federal, state, and local laws and regulations. Intended contract to be awarded on/about October 1, 2010. All responses shall include identification/certification of company size status and any pertinent information pertaining to this sources sought. Responses should include information about your company, prior ambulette experience and indicate if you have prior experience with VA hospitals, a service capability statement, DUNS number, point of contact and the size of your business pursuant to the following questions; (1) Is your business large or small? (2) If small, does your firm qualify as a small, emerging business? (3) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(A) of the Small Business Act? (4) Is your firm a certified Hub Zone firm? (5) Is your firm a woman-owned or operated business? (6) Is your firm a certified Service-Disabled Veteran Owned or Veteran Owned Small Business? Provide a minimum of three "REFERENCES" for same or similar services. This is a market survey for informational and planning purposes only.
